Home My WebLink About000104 AmendmentSERVICE ORDER 1 TO THE CONTRACT FOR SANITARY SEWER SYSTEM CLEANING VIDEOING QETWEEN THE THE CITY OF PORT ANGELES AND PACIFIC CONCRETE SERVICES WW -11 -036 RELATING TO: CONTRACT FOR SANITARY SEWER SYSTEM CLEANING VIDEOING, WW -11 -036 This Service Order No. 1 is made and entered into this l 7 Tit day of ffB 2012, by and between THE CITY OF PORT ANGELES, a non charter code city of the State of Washington, (hereinafter called the "CITY and Pacific Concrete Services, a Washington Corporation (hereinafter called the "CONSULTANT The Service Order is being entered pursuant to the Contract titled "Sanitary Sewer System Cleaning Videoing" WW -11 -036, which was entered into on January 24, 2012. Service Order No.1 contains the first work phase requirements. I SCOPE OF SERVICES The scope of professional services to be performed and the results to be achieved by the CONNTRACTOR for the task identified in the attached Exhibit Al. The CONTRACTOR shall provide and furnish all services and material necessary to accomplish the Task identified in Exhibit Al. II TIME OF PERFORMANCE Service Order No. 1 shall be completed by March 16, 2012. III MAXIMUM COMPENSATION The maximum compensation, including tax, is $15,000 for this Service Order. IV EXHIBITS This Task Order, including its exhibits, constitutes the entire Task Order, supersedes all prior written or oral understandings, and may only be changed by a written agreement by both parties. CITY OF PORT ANGELES: CITY MANAG CONTRACTOR: Div i9�L iii0/t//f TITLE: APPROVED AS TO FORM: CITY ATTORNEY ATTEST: WW- 11-036 Task Order 1, Page 2 2/8/2012 EXHIBIT Al Scope of Services Tasks Contractor shall provide the following services related to Contract for Sanitary Sewer System cleaning Videoing, WW -11 -036: 1. The Contractor shall perform video inspection of the sanitary sewer system in the order provided in Exhibit C1. 2. Video inspection will be performed without prior cleaning of the sanitary sewer system unless it is determined by the contractor that the sewer system condition requires prior cleaning. 3. Sewer system line lengths shown on Exhibit C1 are approximate. Line lengths will be adjusted to reflect measured distances from manhole center to manhole center, or manhole center to cleanout, whichever is appropriate as determined by Contractor or the Project Manager. 4. Sewer system line materials shown on Exhibit C1 shall be corrected if differing material is shown on the videos by the Contractor or by the Project Manager. 5. The Project Manager may change the order of work or location of work for the benefit of the City. 6. The maximum compensation, including tax, for Task 1 is $15,000. A working copy of the Exhibit C1 spreadsheet will be updated daily by the Project Manager. Inspection shall cease when the cost of work (cleaning and inspection) reaches the $15,000 limit. This Service Order does not alter the provisions of the Original Contract dated January 24, 2012.
