AMENDMENT NO. 4 TO THE
AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN
THE CITY OF PORT ANGELES
AND
LINDBERG & SMITH ARCHITECTS
PROJECT PD01-2011
THIS AMENDMENT NO. 4 is made and entered into this t-7 day of Aril , 2015, by
and between the City of Port Angeles, a non-charter code city of the State of Washington,
(hereinafter called the "CITY") and LINDBERG & SMITH ARCHITECTS, INC., (hereinafter
called the "CONSULTANT").
NOW, THEREFORE, in consideration of the representations and the terms, conditions,
covenants and agreements set forth in the original AGREEMENT and this AMENDMENT, the
parties hereto agree as follows:
SECTION 1
The CITY entered into the existing AGREEMENT with the CONSULTANT on April 24, 2012,
(the AGREEMENT). The CITY and the CONSULTANT amended the AGREEMENT on
September 18, 2012, August 20, 2013, and May 6, 2014 (the AMENDMENTS).
SECTION 2
I AMENDED SCOPE OF SERVICES
A. The scope of the professional services is amended to include work as described in the
attached Exhibit A to Amendment 4.
II TIME OF PERFORMANCE
The Added Services Scope of Work in Exhibit A of Amendment 4 shall be completed by
December 31, 2015.

III MAXIMUM COMPENSATION
The CONSULTANT'S total compensation and reimbursement are stated in the attached Exhibit
A to Amendment No. 4. The maximum compensation is increased by$17,400.00 to
$118,746.00. The budget for Amendment No. 4 may be further adjusted by mutual agreement
without an amendment to the Agreement, as long as the maximum compensation of$118,746.00
is not exceeded.
SECTION 3 RATIFICATION
Except as modified herein, the original AGREEMENT and AMENDMENTS are confirmed and
ratified. In WITNESS WHEREOF, the parties hereto have executed this Amendment No. 4 to
the Agreement as of the date and year first written above.

Amendment No. 4 Exhibit A—Added Services Scope of Work and Budget
The following items are to be revised in the design drawings and specifications:
1. Existing equipment has changed. The Fujitsu AC unit in the City Hall Data Center has
been removed and two AC units have been relocated to be in the Data Center. These
changes effect placement of new equipment, changes to phasing and revisions to our
drawings.
2. Existing ductwork serving the City Hall Data Center has been demolished. This
ductwork was to be re-used and will now need to be designed new. An existing AC unit
will need to be relocated and the area this is to be located in will need to be investigated
and designed.
3. HVAC controls design changes will be needed to accommodate the existing equipment
changes.
4. Revise the phasing plan for changes made to the HVAC equipment and the electrical on-
line diagram.
5. Coordination for access control in the Police Department attic area. The City wants the
Uninterrupted Power Supply (UPS) system fenced off for security.
6. Revise the drawings to move the data outlet boxes off the floor. The boxes will need to
have details designed.
7. The City has provided additional direction concerning the electrical power system
requiring changes to be made to the electrical one-line diagrams.
Sub - Total Cost: $9,000.00
8. Gen-Set-2 is to be reconnected for 208Y/120 volt operation at the generator, eliminating
need for a transformer. This work to be accomplished by City of Port Angeles before
contract work begins and will be shown as existing on Hunt Engineering drawings as of
04-02-2015 scope change.
9. Panel X4 remains. The feeder to it is to be replaced with 225A.
Hunt scope: Call for new feeder. Part of current scope.
10. Existing transformer XMR2 is to remain where it is and continue feeding existing
panel XC,to be renamed"OC".
Hunt Scope: Part of current scope.
11. Generator No. 1 circuit breaker replacement,feeders to ATS's, and new ATS's
design is to be per September 26, 2014 design drawings.
12. Generator No. 2 ATS is to be replaced.
13. Revise phasing plan for changes in scope of work and for having Phone and Radio
Room UPS operational by June 30,2015.

14. Assist Owner with Pre-Ordering UPS and any other equipment necessary to have
Phone and Radio Room UPS operational by June 30, 2015.
15. Revise Specifications for equipment Pre-Ordering.
16. Revise Cost Estimate into categories: City to provide guidance for categories by
April 8, 2015 in order to separate City work from Pencom work, and grant
supported work from non-grant supported work.
