SUBJECT: Site Inspections for Lots 3 and 4, Block 199, TPA and Lot 15, Block
199, TPA.
Dear Mr. Quinn:
This office has performed a visual site inspection on the above referenced properties as
requested by the City of Port Angeles. The purpose of this inspection was to assess
possible environmental concerns that may exist on the three subject properties based on
only a visual inspection of the sites. Following is a description of the subject properties:
Lot #3 - 128 East 5th Street, Port Angeles
This lot is located on the south side of East 5th Street. The subject site contains a
two story building which appears to have been constructed for use as a single
family residence. The structure is of wood framed construction and sits on a post
and pier foundation. At the time of the inspection, it appears that the lower floor
may have recently been used as an office and the upper floor appears to have
been used as an apartment. There is no common access stairway between the
floors. The flooring on the lower floor is mostly wood with some areas containing
old sheet vinyl flooring and tiles. The upper floor is carped except for the
bathroom which contains vinyl flooring. The finish on the walls and ceilings is
painted gypsum wallboard or lath and plast~r of various colors with painted wood
trim. The exterior wood siding is painted white. The wood joists, posts and sub
floor are all black in color and mold is suspected. There were no signs of any
heating ducts throughout the structure; the current heat source appears to be
electric baseboard heaters located in the various rooms of the structure, both on
the first and second floors. The structure has two brick chimneys which appear
to have been abandoned in place. Although a structural evaluation was not
requested, it was noted during the inspection that the floors sag and there is
probably differential settlement. The wood staircase leading to the upper floor
also is in very poor condition with some rotten wood.

A small wood-framed shed is located along the east property line approximately
30 feet north of the alley. This structure isin a state of disrepair. The shed
appears to contain only some garbage.

Lot #4 -126 East 5th Street, Port Angeles
This lot is located on the south side of East 51h Street and is just west of Lot No.
3. The lot is empty and is covered in grass. There are no signs of previous
structures on the site. A review of City of Port Angeles records found no records
of construction on the site although good City Building Department records begin
in the 1970's. There were no obvious signs of any underground tanks on the
subject site.
Lot No. 15 - 321 East 6th Street, Port Angeles
Lot No. 15 is located directly south of Lot NO.4 noted above and is separated
from Lot NO.4 by a paved alley. The subject site is covered with grass and a
couple of trees. There are signs of a previous residence on the site with what
appears to be portions of a foundation showing and a sidewalk remaining on the
lot. There is a water meter within the City of Port Angeles Right-Of-Way and
what appears to be a water shutoff valve encased within a 4" nominal concrete
pipe on the site. There were no obvious signs of any underground tanks on the
subject site.
Based on the limited site reconnaissance, there are several items recognized as
possible environmental concerns. Due to the age of the existing structure on Lot No.3,
it is likely that lead based paints were used on the structure and that asbestos containing
materials are found within the structure. These items would require addressing prior to
beginning any new construction, remodel, or demolition activities. An asbestos survey
would be required and any asbestos found would require abatement by a licensed
abatement contractor prior to any construction activities following applicable state laws.
A lead paint survey would be required before demolition debris could be disposed of in
the City of Port Angeles Landfill.
Although not found in the limited site reconnaissance, the possibility exists that
underground storage tanks could be located on any of the three sites. Due to the lack of
records in regards to underground storage tanks, typically heating oil tanks for
single family residences, it is unlikely these tanks would be found except possibly during
site disturbing activities. Since the possibility of tanks does exist, there is a
potential environmental concern. The level of remediation required is dependent upon
the proposed use of the side. The surrounding area appears to be mostly residential in
nature, but no review of records was done to determine if any sites in the area have any
documented environmental concerns such as underground storage tanks or commercial
activities which may create environmental impacts on the three subject lots.
It is also known that there are a number of underground storage tanks along Lincoln
Street which have been identified during research for other projects. These tanks could
have some impacton the subject site although most of these site are probably either far
enough away or are down gradient ofthesubjectsite.There is however some
possibility of ground contamination on the subject sites. A review of public records might
provide additional information pertaining to possible environmental concerns on the
subject site or surrounding neighborhood but it is not within the scope of our services.

Based on our reconnaissance, it is the opinion of this office that the site contains the
typical environmental concerns found on typical residential sites throughout the older
portions of the City of Port Angeles. With the limited scope of work for this project, visual
inspection of the subject sites with no testing or research of records, it cannot be said for
certain that ground contamination, as noted above, will not be found during site
disturbing activities although it does not appear to be a high risk. This report is based on
the limited scope of work requested by the City of Port Angeles and it is not
warranted that the report cannot be superseded by future environmental, legal,
geotechnical or technical developments.

As you requested, I solicited abatement survey prices from local Engineering firms. The only local
company that I could find that does this kind of work is Zenovic & Associates, and they don't even do the
testing in-house. Zenovic's uses a company from Port Townsend. It is my suggestion that we use
Zenovic's proposal to complete the survey that we have requested. The cost & time involved to request
other survey's from Seattle or Bainbridge Island companies would not save the City any money or time.

We just received the Zenovic proposal with a cost of $850 to do the study. I believe that estimate is
reasonable and that Roger's efforts to obtain other proposals have proved fruitless. Therefore, I
recommend that we proceed ahead with Zenovic on the asbestos removal study. What budget account
does it get assigned?

Public Works & Utilities is not responsible for the City Hall structure or parking lots. As I have mentioned
to you in the past Building has taken the lead in the past for demolition of derelict structures. I consider
this to be a similar situation.
Funding needs to be addressed to Yvonne. She has funds available for the demolition the same way
she had funds available for the purchase.

purchasing the 328 E. 5th Street property. Since the property is expected to be a parking lot for City Hall, I

manage any City property, and I am not sure what type of project number we should assign. Do you want

Attached are three estimates for demolition fthe City owned structure at 328 E. 5th Street. Comparison
of the estimates for demolition only (not' cluding permit fees, dump fees, asbestos/lead removal, and
taxes) are Morrison ($3000), RJ Servi s ($3500), and Crescent Development ($3825). With other costs
such as those listed above but excl ed from the estimates, we can expect according to the Morrison
estimate another $2200 plus. Tr a Funston and Keith Bogues know the procedures for asbestos and
lead (hazardous materials) re val and should be able to direct the demolition project. Besides the
demolition permit issued b~ e Building Division, an ORCAA air pollution control permit may be
needed also. I don't beli e the permit fees and the dump fees should be waived per standard fmance
policy. To proceed, weed authorization to spend the necessary funds for first the asbestos/lead
removal (for which e don't have an estimate but which is a requirement) and second for the remaining
demolition contra and fees.

Enter all applicable information accurately and completely. Incomplete Notices of Intent will be returned.
Box A. Chcek all appropriate project type.
Box B. Enter the legal property owner information.
Box C. Enter the asbestos contractor information, or "not applicable" eN/A) for demolition prqjects when no asbestos~
containing materials are present. For a Residential Dwelling asbestos removal project, list the property owner doing the
asbestos removal (include property owner's mailing address).
Box D. Enter the asbestos removal or demolition site address. For a Residential Dwelling asbestos removal project where the
property owner's mailing address differs from the site address listed in Box D, a letter must accompany the Notice of Intent
verifYing that the structure located at the site address is currently being used a.~ the property owner's domicile. Multiple
asbestos/demolition projects involving more than one StructUrfl must be submitted in accordance with the ORCAA Regulation
1, Section 14.05(a)(S).
.Residential dwelling (ORCAA Article 14, Section 14.03 Definitions) means any non-multiple unit building
containing space for uses such as living, sleeping preparation of food, and eating that is owned, used, occupied, or
intended or designed to be occupied by One family as their domicile. This term includes houses, mobile homes, trailers,
houseboats, and houses with a "mother~in~law apartmenf' or "guest room:' This ferm does not include structures that
are demolished or renovated as part of a commercial or public project. Nor does this term include any mixed-use
building, structure, or installation that contains a residential unit, or any building that is leased or used as a rental.
Box E. Enter the asbestos survcy information or check material presumed (for asbestos projects only). An AHERA building
inspector must conduct an asbestos survey prior to any demolition project (i.e., the wrecking, razing, leveling, dismantling or
burning of a strucmre, making the Struclurc pennanently uninhabitable or unusable). A copy of the survey results shall
accompany the application.
Box F. Enter the dcmolition project information. If the Struclure is to be used in a training f11'e, list the fire department
responsible for conducting the bum as the demolition contractor in Box J. If the propcrty owner has been ordered to perform a
demolition by a government official, submit a copy of the order from the appropriate official.
Box G. Enter all asbestos project information or enter UN/A" for demolition projects with no asbestos removal required. All
multiple structure asbestos projects must be submitted with a work plan. map of the structures. comvlete site address. Me and
amount of asbestos material to be removed from each structure. AHERA asbestos survey. and work schedule.
Box 8. Check appropriate project categories in boxes 1 through 8. Asbestos project fee does not include dcmolition project.
An additional $25.00 is required for a demolition project. Emergency notifications must be submitted with a letter from the
property owner explaining the necessity for thc emergency. Emergency demolition notifications must be submitted with a
leuer from an authorized government official or a licensed structural engineer documenting that the structure is in imminent
danger of collapse.

appropriate analytical requirements for waste characterization. Ecology Publication #91-30
(Revised April 1994) "Guidance for Remedialion of Petroleum Contaminated Salls" shall also be
used to characterize pelroleum contaminated soils from UST releases. Submit all laboratory
analytical results, ONOC data, and Chain of Custody sheets along with this application.

to be removed by a certified abatement contractor which follows the rules
of the EPA and governed by Olympic Region Clean Air Agency.
This report is not a guarantee that all suspect of AC.B.M. were found.
The possibility of concealed material exist and may be found during,
demolition. Please contact NWAC for further inspection if needed.
After the facility is completely cleaned out a walk through and inspection
is required by the original AHERA building inspector (NW Asbestos) after
abatement, then a copy of the letter certifying that abatement has been
completed needs to be received by the City of Port Angeles Permit Center
and Olympic Region Clean Air Agency.

You asked me to find out what are contract covered for the house demo at 328 East 5th.
I am sorry it has taken me so long to get back to you. The cities contract with Morrison Excavating was to
demo and remove the house at 328 E 5th Street, it did not include site clean up and soil restoration and
leveling. So it looks like the city will need to contract with someone or have city crews clean, fill, grade &
seed the site.

Attached are three estImates for demolItIOn of the City owned structure at 328 E. 5th Street. Companson
of the estimates for demolItIOn only (not mcludmg permIt fees, dump fees, asbestos/lead removal, and
taxes) are Momson ($3000), RJ ServIces ($3500), and Crescent Development ($3825) With other costs
such as those lIsted above but excluded from the estimates, we can expect accordmg to the Mornson
estrmate another $2200 plus. Trema Funston and KeIth Bogues know the procedures for asbestos and
lead (hazardous matenals) removal and should be able to dIrect the demolItIOn project. BeSIdes the
demolItIOn permIt Issued by the Buildmg DIVIsIOn, an ORCAA au pollutIOn control permIt may be
needed also I don't belIeve the permIt fees and the dump fees should be Waived per standard finance
polIcy To proceed, we need authonzatIOn to spend the necessary funds for fust the asbestosllead
removal (for wruch we don't have an estrmate but WhICh IS a reqUIrement) and second for the remaImng
demolItIOn contract and fees.
