plan submittal requirements if you have questions.
VALUATION OF CONSTRUCTION: In all cases, a valuation amount must be entered by the applicant. This figure will be reviewed
and may be revised by the Building Division to comply with cun'ent fee schedules. Contact the Pernnt Coordinator at 417 -4815 for assistance.
PLAN CHECK FEE: IF a plan check fee is due it must be submitted at the time the building permit application and construction plans are
submitted. All other permit fees are due at the time ofpernnt issuance.
EXPIRATION OF PLAN REVIEW: If no permit is issued within 180 days of the date of application, the application will expire. The
Building Official can extend the time for action by the applicant up to 180 days upon written request by the applicant (see Section R1 05.3.2
of the International BuildinglResidential Code, 2003). No application can be extended more than once.

This office performed special inspections and structural observation during the
construction of the bridge noted above. The following special inspections were
performed:
a. Concrete test specimens were taken during the placement of concrete for the
precast bridge deck. Enclosed are final concrete test results showing that the
compressive strength of the concrete meets the requirements of the design.
b. Reinforcing steel was inspected for conformance with the approved plans and
approved prior to placement of concrete.
c. During the driving of the steel pipe piles. All pipe piles were driven to a minimum
capacity of 25 tons. Depth of piles varied from 11' to 14'.
Based on the inspections it is the opinion of this office that the work, to the best of the
inspector's knowledge, is in general conformance to the approved plans, specifications,
and applicable codes.

Dear Mr. Haehnlen:
I have examined the plans and calculations for the proposed bridge at Peabody Creek
R.V. Park. The following items of concern were found during the review and should be
addressed prior to issuance of a permit on the project:
1. The design of the vehicle barrier curb shall meet load and location requirements
of AASHTO or the Uniform Building Code. Revised plans and calculations shall
be provided, as needed, to meet these requirements. (UBC requires vehicle
barriers to withstand a 6000 lb. point load at a point 18" above grade)
2. Based on the plans and calculations provided it appears that the malleable
washers on the 10 x 10 curb rail are not of adequate size to prevent crushing of
the wood barrier under the tension load.
3. The bridge design does not appear to account for lateral loads imposed on the
bridge from wind, seismic, or braking forces. Are lateral forces transferred into
the soil at eact;pf the bridge?

4. With pedestrian guardrail posts installed at six foot on center to align with every
other curb bolt it is not reasonable to assume that the load is transferred out to
the curb bolts which do not align with posts. Please revise plans and calculations
as necessary.
5. Calculations shall be provided to show that the actual working stress in the
concrete deck slabs does not exceed the allowable working stress in the
concrete.
6. Special inspections required on the project per USC Chapter 17. Clearly Indicate
on the plans all items requiring special inspection (concrete, piles, etc.).

1. TIMING LIMITATIONS: The project may begin Immediately and shall be completed by September 30, 2002.
2. NOTIFICATION REQUIREMENT: The pennittee or contractor shall notify the Area Habitat Biologist (ARB) listed
below of the project start date. Notification shall be received by the ARB at least three working days prior to the start
of construction activities. The notification shall include the permittee's name, project location, starting date for work,
and the control number for this Hydraulic Project Approval.
3. Work shall be accomplished per plans and specifications submitted to the Washington Department ofFish and WildlIfe,
except as modified by this Hydraulic Project Approval. These plans reflect design criteria per Chapter 220-11 0 WAC.
These plans reflect mitigation procedures to significantly reduce or eliminate impacts to fish resources. A copy of these
plans shall be available on site during construction.
4. Excavation for and placement of the foundation and superstructure shall be outside the ordinary high water line.
5. Excavation for the footings shall be completely separated from the stream by placing the footings landward of the top of
the bank.
6. The bridge structure shall be placed in a manner to minimize damage to the streambed and banks.
7. The bridge shall be constructed to pass the 100-year peak flow with consideration of debris likely to be encountered.
8. Abutments, piers, piling, sills, approach fills, etc., shall not constrict the flow and cause any appreciable increase (not
to exceed 0.2 feet) in backwater elevation (calculated at the 100-year flood) or channel-wide scour, and shall be aligned
to cause the least effect on the hydraulics of the stream.
9. Riprap materials used for structure protection shall be clean, angular rock, which shall be installed to withstand the
IOO-year peak flow.

10. Structures containing concrete shall be sufficiently cured prior to contact with water to avoid leaching. Fresh concrete
shall not be allowed to come into contact with state waters.
11. Removal of the existing structure shall be accomplished so the structure and associated material does not enter the
stream. Material shall be disposed of so it will not re-enter the stream.
12. Removal shall be accomplished by mechanical means. This Hydraulic Project Approval does not authorize blasting.
13. Equipment used for this project shall not enter or operate below the ordinary high water line.
14. Equipment used for this project shall operate stationed on the stream banks.
15. Equipment crossings of the stream are not authorized by this lIP A.
16. All treated wood shall be professionally treated and completely cured prior to installation below the ordinary high water
line to minimize leaching into the water or substrate. The use of wood treated with creosote or pentachlorophenol is not
authorized.
17. All lumber to be used for the project shall meet or exceed the standards established in "Best Management Practices For
the Use of Treated Wood in Aquatic Environments" developed by the Western Wood Preservers Institute, dated July
1996.
18 . Wooden components which will be in contact with the water shall not contain creosote or pentachlorophenol. This shall
include pilings, beams, structural supports, and decking. These components shall remain free of these toxic substances
for the duration of their functional lives.
19. Every effort shall be taken during all phases of this project to ensure that sediment-laden water is not allowed to enter
the stream.
20. Erosion control methods shan be used to prevent silt-laden water from entering the stream. These may include, but are
not limited to, straw bales, filter fabric, temporary sediment ponds, check dams of pea gravel-filled burlap bags or other
material, and/or immediate mulching of exposed areas.
21. All waste material such as construction debris, silt, excess dirt or overburden resulting from this project shall be
deposited above the limits of flood water in an approved upland disposal site.
22. Ifhigh flow conditions that may cause siltation are encountered during this project, work shall stop until the flow
subsides.
23. Extreme care shall be taken to ensure that no petroleum products, hydraulic fluid, fresh cement, sediments, sediment-
laden water, chemicals, or any other toxic or deleterious materials are allowed to enter or leach into the stream.
24. Fresh concrete or concrete by-products shall not be allowed to enter the stream at any time during this project. All

This Hydraulic Project Approval (HP A) pertains only to the provisions of the Fisheries Code (RCW 77 .55 - formerly RCW
75.20). Additional authorization from other public agencies may be necessary for this project.
This HP A shall be available on the job site at all times and all its provisions followed by the penmttee and operator(s)
performing the work.
This HP A does not authorize trespass.
The person(s) to whom this HPA is issued may be held liable for any loss or damage to fish life or fish habitat which results
from failure to comply with the provisions of this lIP A.
Failure to comply with the provisions of this Hydraulic Project Approval could result in a civil penalty of up to one hundred
dollars per day or a gross misdemeanor charge, possibly punishable by fine and/or imprisonment.
All HPAs issued pursuant to RCW 77.55.100 or 77.55.200 are subject to additional restrictions, conditions or revocation if
the Deparbnent ofFish and Wildlife determines that new biological or physical information indicates the need for such

challenged. The time period for requesting a formal appeal is suspended during consideration of a timely informal
appeal. If there has been an informal appeal, the deadline for requesting a fonnal appeal shall be within 30-days of the
date of the Department's written decision in response to the informal appeal.

The slabs are designed for two lanes of standard HS-20-44 trucks with full impact allowance,

The concrete to be used on this project is fc= 3500 psi in site-cast concrete and fc = 4000 psi in the
precast slabs. All concrete shall have 4-7% entrained air, 3/4" min. large aggregate size. The slump
shall not exceed 4.5" as sampled and measured at the truck chute at any time during placement. Concrete
stablizers may be used to delay hydration during transit to the project, with specific approval of the Engineer.

When handling the slabs, place spacers and stickers so that no portion of the slabs canti/ever along the longitudinal
length of the slab. If such handling is determined unavoidable, add a top mat of steel with (1 #6 bars x 24' 8" long
in the longitudinal direction, evenly spaced across the width of the slab (#4 transverse at 18'" ctrs may be used to
position the bars).
Grout used in pinholes and railing dowels shall be Target Portland Expanding Grout, available from
Target Products (604) 420-3620. If other grout is to be used, send complete material data sheets
to the Engineer for approval prior to use.

The reinforcing steel sha/l meet requirements of ASTM A615, Grade 60. Splices shall be as follows:
#4 bar @ 18", #5 bar @ 22", #6 bar. No splices allowed in the #9 bars.
The piles sha/l be PP 12.750 x 0.375 (A36) pile driven to 25 ton or refusal. The pi/es shall be driven
open ended, The Engineering News formula shall be used to equate set to capacity. Provide
the Engineer with pile hammer data before beginning pi/e driving.
All lumber in the 10 x 10 rai/s and 6 x 10 risers shall be Douglas fir -larch Post and Timbers, NO.1 grade.
All lumber in the rai/posts and 2 x 6 rail elements shall be Douglas fir-larch No. 1 grade. Treatment shall be
with ACZA (ammoniated copper zinc arsenate) aeording to AASHTO M133.
All bolts and nuts shall meet the requirements of ASTM A307.
