Extinguishers must be mounted, with the top no more than 5'
off the floor. Suggested extinguisher placement is
adjacent to an exit.

The proposal will result in construction of an
office/storage structure in the CO zone for total lot
coverage of 13%. No land use issues are anticipated as
designed.
Trench safety per applicable laws. temp erosion control and
surface restoration responsibility of applicant. Contact
City inspector prior to start of work @ 360 417-4831. No
attachment of stormwater leaders, foundation drains, yard
drains, or any other CSO contribution is allowed.
A separate Right of Way Construction permit is required for
all work in the right of way, application must include all
contractors and sub-contractors, each must provide the City
with a certificate of insurance listing the City as an
additional insured.
All connections to City storm drain facilities require an
inspection by Public Works and Utilities Engineering prior
cover. Notice will be given 48 hours in advance of
commencing work.
Sanitary sewer connection inspection is required by
Public Works prior to back fill of ditch. 24 hour advance

Construct driveway and Sidewalks to City Standards.
Concrete with exposed aggregate or other non-standard
finishes(including colors or dyes)are not allowed in the
City road right of way. Broom finish only. An inspection by
Public works Engineering is required prior to pouring

PURPOSE
The purpose of this report is to address how the proposed stormwater design meets the
requirements of the City of Port Angeles Urban Services Standards and Guidelines. The
report will provide existing site conditions, proposed development, City of Port Angeles
stormwater requirements, drainage area summary, flow control calculations and a
maintenance schedule.
EXISTING SITE CONDITIONS
The project site is located at 401 East Orcas Avenue., Port Angeles, WA; parcel number
06-30-10-509090. The total area of the parcel is 11,761 square feet. The site currently
has three existing outbuildings and a gravel driveway crossing a portion of the site. The
remainder of the site is lawn and landscaping. The site also included a 1,321 square
foot home which was demolished in preparation for this project. The parcel is bordered
to the north by parcel 06-30-10-509080, to the east by parcels 06-30-10-502120 and 06-
30-10-502130, to the south by Orcas Avenue and to the west by South Peabody Street.
The parcel and surrounding properties are zoned CO- Commercial Office.
The topography of the site and surrounding properties slopes consistently down to the
northeast at approximately 4-5%. The on-site soil is classified as Clallam - Hoypus
according to the Custom Soil Resource Report for Clallarn County Area, Washington
(2006). Clallarn - Hoypus soil is a gravely sandy loam, but generally has a restrictive
glacial till layer found between 20 to 40 inches below the surface. Ground water is
typically perched above this restrictive layer during the winter months.
PROPOSED DEVELOPMENT
The proposed development will construct a 1,536 square foot storage building with an
attached 160 square foot carport, and add approximately 1,563 square feet of concrete
to the parking lot and approximately 182 square feet of concrete sidewalk. In total the
development Will convert approximately 2,275 square feet of pervious lawn and
landscaping surface to impervious roof, asphalt and concrete surface. See drainage
area summary below for additional information.
CITY OF PORT ANGELES STORMWATER REQUIRMENTS
The proposed project disturbs less than 1 acre of land and therefore falls under the
requirements of Section 5.05.02 (Small Project Drainage Plans)of the City of Port
Angeles Urban Service Standards and Guidelines.
Section 5.05.02 requires a flow control system, to be designed in accordance with
Section 5.04.07, if the site drains to a storm system which is already at capacity.
According to Section 5.04.07 the development of the site doesn't meet any of the criteria
that would require flow control, but a flow control facility is still being proposed due to
consistent flooding that occurs during large storm events along Lauridsen Boulevard.
Section 5.05.02 requires a runoff treatment facility, designed in accordance with Section
5.04.06, if it is likely the project Will have a negative impact on water quality in a local
stream, wetland or salt water body. The addition of Pollution Generating Impervious
Surface(PGIS) is small enough as to not meet any of the thresholds listed within Section
5.04.06, so a treatment facility is not required or proposed. See drainage area summary
below for PGIS area.

FLOW CONTROL CALCULATIONS
Flow control is not required for this project according to Table 2 in Section 5.04.07.02,
but a detention/flow control system is being proposed due to the overloaded storm
system along Lauridsen Boulevard. The predeveloped and develop site conditions were
modeled using the Rational Method based on the areas shown in the drainage area
summary above. The detention pipe and outlet structure were sized so the developed
outfall from the site matches the predeveloped outfall for the 2 year and 25 year storm
events. Drainage calculations can be found in Appendix A.
OPERATION AND MAINTENANCE
A maintenance schedule for the on-site drainage facility has been completed to satisfy
the requirements within Section 5.05.02 of the City of Port Angeles Urban Services
Standards and Guidelines. The maintenance schedule is included in Appendix B.

PURPOSE
The purpose of this report is to provide stormwater facility operation and maintenance protocols
for the stormwater facilities at 401 East Orcas Avenue. The maintenance report provides
background information for the development, identifies all stormwater facilities to be maintained,
names the entity legally responsible for maintenance, grants City of Port Angeles personnel
access for annual inspections, details requirements for maintaining records and provides a
maintenance checklist and log.
BACKGROUND
The total area of the parcel is 11,761 square feet. The parcel currently has three existing
outbuildings and a gravel driveway with the remainder of the site being lawn and landscaping.
The site did include a 1,321 square foot home which was removed in anticipation of this project.
The project consists of constructing a 1,536 square foot storage building with an attached 160
square foot carport, construction of concrete sidewalk, construction of a concrete driveway, and
the installation of an 18" diameter stormwater detention pipe with an outlet control structure and
associated 6" diameter piping. The project site is located at 401 East Orcas Avenue, Port
Angeles, WA; parcel number 06-30-10-509090.

MAINTENANCE RESPONSIBILITY
The North Olympic Library System will be the sole entity legally responsible for the continual
operation, maintenance and repair of the on-site stormwater facilities.
The City of Port Angeles will be the sole entity legally responsible for the maintenance of the
newly installed type 2 catch basin located in Peabody Street.
CITY INSPECTIONS
The City of Port Angeles shall be allowed access to the site for an annual inspection of all
permitted stormwater facilities.
RETENTION OF RECORDS
The person legally responsible for the facilities shall maintain all records of maintenance
inspections, repairs, warning letters, and notices of Violations. These records shall be made
available for viewing to the City of Port Angeles within 7 days of request.

MAINTENANCE
Proper maintenance is vital for adequate functioning of the stormwater facilities. The
stormwater facilities will have biannual inspections. See Table on pages 4 and 5 for an
inspection checklist. Make photocopies of the checklist as necessary keeping the attached
sheets as a clean original copy.
MAINTENANCE AND REPAIR LOG
Record any required maintenance or repair on the log sheet on page 6. Make photocopies of
the log sheet as necessary keeping the attached sheet as a clean original copy.
MAINTENANCE TIMELINE
This covenant requires that when defects to the stormwater system are found during the
biannual inspection the needed repair shall be completed within 6 months.

BIANNUAL MAINTENANCE INSPECTION CHECKLIST

Inspect pavement surface of accumulated sediment and debris.
Parking -Pavement sweeper to sweep parking lot clear of accumulated sediment and
debris.
Remove lid and inspect sumps.
-Rernove excess silt, debris and vegetation.
-Look for any evidence of oil, gasoline, contaminates or pollutants.
Coordinate removal/cleanup with local water quality response agency.
Inspect basin for any defects.
-Repair any defects that could allow water in or out of basin in any way other
than as designed. Repair any cracks or holes. Significant damage may

Inspect frame and grate.
-Frame should be sifting flush on the riser rings or top slab and firmly
attached.
-Check lid to make sure that it can be removed by one maintenance person
by applying normal lifting pressure. Cleanout frame of sediment and debris.

-Pipes should be securely grouted to catch basin wall without cracks or
holes. Regrout pipes to catch basin wall to eliminate all cracks and holes.
Remove lid and inspect sumps.
-if more than 4"of sediment accumulated on bottom, employ vacuum truck to
remove standing water, excess silt, debris and vegetation.
-Look for any evidence of oil, gasoline, contaminates or pollutants.
—Coordinate removal/cleanup with local water quality response agency.
Inspect orifice holes.
-Rernove any sediment, vegetation, or debris that is or could soon block flow
Outlet through orifice holes.
Control Inspect Overflow Pipe.
Structure -Remove any trash and/or debris blocking (or having the potential of
blocking)the overflow pipe.
Inspect structure for any defects.
-Repair any defects that could allow water in or out of control structure.
Significant damage may require control structure to be replaced.
Check inlet and outlet pipes.
-Pipes should be securely connected to structure. If, by visual inspection, it
can be seen that the pipes aren't connected properly than the pipes W411 need
to be reconnected to the bell fittings and sealed with grout or joint sealant.

Remove outlet structure lid and inspect pipe.
-if more than 4" of sediment accumulated on bottom, employ vacuum truck to
remove standing water, excess silt, debris and vegetation.
-Look for any evidence of oil, gasoline, contaminates or pollutants.
--Coordinate removal/cleanup with local water quality response agency.
Detention Inspect pipe for any defects.
Pipe -Repair any defects that could allow water in or out of pipe in any way other
than as designed. Repair any cracks or holes.
Check inlet and outlet pipes.
-Pipes should be securely grouted to catch basin wall without cracks or holes.
Regrout pipes to catch basin wall to eliminate all cracks and holes.
General Document observations of the presence of floating materials, suspended solids,
oil and grease, discoloration, turbidity and odor in stormwater discharges.
General Update Stormwater Maintenance Manual to reflect current conditions.
Inspect the entire site to determine whether there is/are any unpermitted
General non-stormwater discharge to storm drains or receiving waters. Report non-
permitted discharges to the City Stormwater Hotline at(360)417—4830.
