PROJECT MANUAL
POWER POLE REPLACEMENTS
SCOPE OF WORK
Replace existing wood poles at various locations in the City of Port Angeles (COPA) power
distribution system. Work includes providing all labor and equipment required. Material
will be provided by the City of Port Angeles. Poles, crossarms, and other hardware
removed shall be delivered to the City of Port Angeles storage yard. Installation shall be
performed according to the City of Port Angeles Construction Standards and other details
included in these documents. . Refer to detailed drawings in Appendix A of this document.
All work shall be completed according to requirements of the 2007 edition of the National
Electrical Safety Code (NESC), plus applicable OSHA and WISHA rules. Section 1 of the
2006 Washington State Department of Transportation Standard Specifications for Road,
Bridge, and Municipal Construction shall be incorporated into this project manual by
reference.
Bid price shall 5e a lump sum to include all work to complete the installation, including the
furnishing of all labor, tools, and equipment needed, and removal of material noted to be
removed. Poles, crossarms, and other pole hardware will be provided by the City.
ADDITIONAL REQUIREMENTS
1. Sealed bids will be received by the Public Works and Utilities Director until 2:00 PM,
Tuesday, February 26, 2007 and will be opened and read in the Public Works &
Utilities Conference Room, Port Angeles City Hall, 321 East Fifth Street, Port
Angeles, W~shington 98362. Questions may be directed to Craig Fried, Contract
Specialist, at (360) 417-4541.
2. Bids must be sealed with the outside of the envelope marked "BID OPENING DATE
February 26,2008, CONTRACT NO. WF-65680". The name and address of the
bidder shall also appear on the outside of the envelope. Bids shall be addressed:

3. The most recently issued Washington State Prevailing Wage Rates for Clallam
County, a copy of which is attached to this Project Manual, shall apply to this
Contract. Contractor shall file a "Statement of Intent to Pay Prevailing Wages" with
Washington Department of Labor and Industries.

4. The bid may be awarded to the lowest responsible bidder from the group of Small
Works Roster contractors asked to bid on this work. If the bid is awarded, the
attached Contract will then be signed, requiring the Contractor to meet the
requirements in the Contract. No bidder will be permitted to withdraw its bid
between the closing time for receipt of bids and the execution of a Contract, unless
the award is delayed for a period exceeding sixty (60) calendar days.
5. Bids shall be received no later than 2:00 pm, February 26, 2008.
6. Because this Contract is estimated to be greater than $35,000, a bid bond,
performance and payment bond, and retainage will be required for this work.
A. The bid bond must be executed 'by the bidder and the surety company. The
amount of the bid bond shall not be less than 5% of the total bid amount and may
be shown in dollars or on a percentage basis.
B. The successful bidder will need to provide a Performance and Payment
bond, to be executed by the successful bidder and their surety. The amount of the
bond must be equal to the total bid amount.
C. A retainage of 5% will be withheld from payment applications until all
deficiencies identified during the final inspection are corrected.
7. Contractor shall provide workers that are "Qualified Electrical Workers" as defined
by OSHA 1910.269, WAC 296-45, and members of the International Brotherhood of
Electrical Workers, with the training required by NESC Section 44.
8. A copy of the Contractor's Certificate of Registration must be submitted with the bid.
9. Contractor is responsible for all permits and fees required for completion of the
described work. City electrical and building permits are not required. Right-of-Way
use permits will be required to block any street, unless f1aggers are used.
10. The performance period for this work is 120 calendar days.
11 . Work shall be completed in compliance with the attached drawings and
specifications, including the following appendicies to this project manual:

PUBLIC WORKS
CONTRACT
This Contract is made and entered into in duplicate this ~day of ~. 0 ,2008, by and
between the City Of~. a (fn-chartl code city of the State of Was hi gton, hereinafter referred
to as "the City", and _ V r.tJ4p., hereinafter referred to as "the Contractor".
WITNESSETH:
Whereas, the City desires to have certain public work performed as hereinafter set forth, requiring
specialized skills and other supportive capabilities; and
Whereas, the Contractor represents that it is qualified and possesses sufficient skills and the
necessary capabilities to perform the services set forth in this Contract.
NOW, THEREFORE, in consideration of the terms, conditions, and agreements contained herein,
the parties hereto agree as follows:
I . Scope of Work.
The Contractor shall do all work and furnish all tools, materials, and equipment in order to
accomplish the following project:

in accordance with and as described in this Contract and the Project Manual, which include the attached
Description of Work, and shall perform any alterations in or additions to the work provided under this
Contract and every part thereof. The Contractor shall provide and bear the expense of all equipment,
work, and labor of any sort whatsoever that may be required for the transfer of materials and for
constructing and completing the work provided for in this Contract, except as may otherwise be provided
in the Project Manual.
2. Time for Performance and Liquidated Damages.
a. Time is of the essence in the performance of this Contract and in adhering to the time
frames specified herein. The Contractor shall commence work within 10 working days of
Notice to Proceed, and said work shall be substantially completed within 120 calendar
days of the Notice to Proceed, unless a different time frame is expressly provided in
writing by the City.
b. If said work is not substantially complete within 120 calendar days of the Notice to
Proceed, the Contractor may be required at the City's sole discretion to pay to the City
liquidated damages of $100 for each and every day said work remains uncompleted after
the expiration of the specified time, unless an extension is granted by the City.
3. Compensation and Method of Payment.

b. Should a court of competent jurisdiction determine that this Contract is subject to
RCW 4.24.115, then, in the event of liability for damages arising out of bodily injury
to persons or damages to property caused by or resulting from the concurrent
negligence of the Contractor and the City, its officers, officials, employees, and
volunteers, the Contractor's liability hereunder shall be only to the extent of the
Contractor's negligence. It is further specifically and expressly understood that the
indemnification provided herein constitutes the Contractor's waiver of immunity
under Industrial Insurance, Title 51 RCW, solely for the purposes of this
indemnification. This waiver has been mutually negotiated by the parties. The
provisions of this section shall survive the expiration or termination of this Contract.
7. Insurance Requirements.
The Contractor shall procure and maintain for the duration of the Contract, insurance against
claims for injuries to persons or damage to property which may arise from or in connection with the
performance of the work hereunder by the Contractor, their agents, representatives, employees or
subcontractors. Failure by the Contractor to maintain the insurance as required shall constitute a
material breach of contract upon which the City may, after giving five working days notice to the
Contractor to correct the breach, immediately terminate the Contract or at its discretion, procure or
renew such insurance and pay any and all premiums in connection therewith, with any sums so
expended to be repaid to the City on demand, or at the sole discretion of the City, off set against
funds due the Contractor from the City.
a. Minimum Scope of Insurance
The Contractor shall obtain insurance of the types described below:
1. Automobile Liability insurance covering all owned, non-owned, hired and
leased vehicles. Coverage shall be written on Insurance Services Office
(ISO) form CA 00 01 or a substitute form providing equivalent liability
coverage. If necessary, the policy shall be endorsed to provide contractual
liability coverage.
11. Commercial General Liability insurance shall be written on ISO occurrence
form CG 00 Oland shall cover liability arising from premises, operations,
stop gap liability, independent contractors, products-completed operations,
personal injury and advertising injury, and liability assumed under an insured
contract. The Commercial General Liability insurance shall be endorsed to
provide the Aggregate Per Project Endorsement ISO form CG 25 03 11 85.
There shall be no endorsement or modification of the Commercial General
Liability insurance for liability arising from explosion, collapse or
underground property damage. The City shall be named as an insured under
the Contractor's Commercial General Liability insurance policy with respect
to the work performed for the City using ISO Additional Insured
endorsement CG 20 10 10 01 and Additional Insured-Completed Operations
endorsement CG 20 37 10 01 or substitute endorsements providing
equivalent coverage.

lll. Workers' Compensation coverage as required by the Industrial Insurance
laws of the State of Washington.
IV. Builders Risk insurance covering interests of the City, the Contractor,
Subcontractors, and Sub-contractors in the work. Builders Risk insurance
shall be on a all-risk policy form and shall insure against the perils of fire and
extended coverage and physical loss or damage including flood and
earthquake, theft, vandalism, malicious mischief, collapse, temporary
buildings and debris removal. This Builders Risk insurance covering the
work will have a deductible of $5,000 for each occurrence, which will be the
responsibility of the Contractor. Higher deductibles for floor and earthquake
perils may be accepted by the City upon written request by the Contractor
and written acceptance by the City. Any increased deductibles accepted by
the City will remain the responsibility of the Contractor. The Builders Risk
insurance shall be maintained until fmal acceptance of the work by the City.
b. Minimum Amounts of Insurance
The Contractor shall maintain the following insurance limits:
1. Automobile Liabilitv insurance with a minimum combined single limit for
bodily injury and property damage of $1,000,000 per accident.
11. Commercial General Liability insurance shall be written with limits no less
than $3,000,000 each occurrence, $3,000,000 general aggregate and a
$3,000,000 products-completed operations aggregate limit.
lll. Builders Risk insurance shall be written in the amount of the completed value
of the project with no coinsurance provisions.
c. Other Insurance Provisions
The insurance policies are to contain, or be endorsed to contain, the following provisions for
Automobile Liability and Commercial General Liability insurance.
1. The Contractor's insurance coverage shall be primary insurance as respect to
the City. Any insurance, self-insurance, or insurance pool coverage
maintained by the City shall be in excess of the Contractor's insurance ano
shall not contribute with it.
11. The Contractor's insurance shall not be cancelled or reduced as to coverage
by either party, except after forty-five (45) days prior written notice by
certified mail, return receipt requested, has been given to the City.
d. Acceptability of Insurers
Insurance is to be placed with insurers with a current A.M. Best rating of not less than
A:VII.

e. Verification of Coverage
The Contractor shall furnish the City with original certificates and a copy of the amendatory
endorsements, including but not necessarily limited to the additional insured endorsement,
evidencing the Automobile Liability and Commercial General Liability insurance of the
Contractor before commencement of the work. Before any exposure to loss may occur, the
Contractor shall file with the City a copy of the Builders Risk insurance policy that includes
all applicable conditions, exclusions, definitions, terms and endorsements related to this
project.
f. Contractor's Insurance For Other Losses
The Contractor shall assume full responsibility for all loss or damage from any cause
whatsoever to any tools, Contractor's employee owned tools, machinery, equipment, or
motor vehicles owned or rented by the Contractor, or the Contractor's agents, suppliers or
contractors as well as to any temporary structures, scaffolding and protective fences.
g. Waiver of Subrogation
The Contractor and the City waive all rights against each other, any of their Subcontractors,
Subcontractors, agents and employees, each of the other, for damages caused by fire or other
perils to the extent covered by Builders Risk insurance or other property insurance obtained
pursuant to the Insurance Requirements Section of this Contract or other property insurance
applicable to the work. The policies shall provide such waivers by endorsement or
otherwise.
8. Compliance with Laws.
a. The Contractor shall comply with all applicable federal, state, and local laws,
including regulations for licensing, certification, and operation of facilities and
programs, and accreditation and licensing of individuals, and any other standards or
criteria as set forth in the Project Manual.
b. The Contractor shall pay any applicable business and permit fees and taxes which
may be required for the performance of the work.
c. The Contractor shall comply with all legal and permitting requirements as set forth in
the Project Manual.
9. Non-discrimination.
The parties shall conduct their business in a manner which assures fair, equal and
nondiscriminatory treatment of all persons, without respect to race, creed, color, sex, Vietnam era
veteran status, disabled veteran condition, physical or mental handicap, or national origin, and, in
particular:
a. The parties will maintain open hiring and employment practices and will welcome
applications for employment in all positions from qualified individuals who are
members of the above-stated minorities.

b. The parties will comply strictly with all requirements of applicable federal, state or
local laws or regulations issued pursuant thereto, relating to the establishment of
nondiscriminatory requirements in hiring and employment practices and assuring the
service of all patrons and customers without discrimination with respect to the
above-stated minority status.
10. Assignment.
a. The Contractor shall not assign this Contract or any interest herein, nor any money
due to or to become due hereunder, without first obtaining the written consent of the
City, nor shall the Contractor subcontract any part of the services to be performed
hereunder without first obtaining the consent of the City.
b. ~he" Cpntractor hereby assi~s to the City any and all claims for overcharges
:resulting from antitrust violations as to goods and materials purchased in connection
with this Contract, except as to overcharges resulting from antitrust violations
commencing after the date of the bid or other event establishing the price of this
Contract. In addition, the Contractor warrants and represents that each of its
suppliers and subcontractors shall assign any and all such claims for overcharges to
the City in accordance with the terms of this provision. The Contracto'r'further
agrees to give the City immediate notice of the existence of any such claiin.
11. Contract Administration.

12. Interoretation and Venue.
This Contract shall be interpreted and construed in accordance with the laws of the State of
Washington. The venue of any litigation between the parties regarding this Contract shall be
Clallam County, Washington.

SECTION 16315 - OVERHEAD ELECTRICAL DISTRIBUTION
PART 1 - GENERAL
1.1 SUMMARY
A. This Section includes medium- and secondary-voltage, overhead electrical power
distribution, upgrading of existing overhead electrical power distribution, and the
following:
1. Conductors, connectors, and splices.
2. Wood and fiberglass poles and crossarms.
3. Hardware and accessories.
4. Surge arresters.
5. Cutouts, switches, and fuses.
1.2 DEFINITIONS
A. ACSR: Aluminum conductor, steel reinforced.
B. BIL: Basic impulse level, stated in kilovolts.
C. COPA: City of Port Angeles Public Works and Utilities Department
D. RUS: Department of Agriculture, Rural Utilities Service.
E. NESC: National Electrical Safety Code, IEEE Standard C2-2007.
F. Sag: The distance measured vertically from a conductor to the straight line joining its
two points of support, measured at the midpoint of the span, unless otherwise
indicated.
1. Final Sag: The sag of a conductor under specified conditions of loading and
temperature applied after it has been subjected, for an appreciable period, to the
loading prescribed for the. loading district in which it is situated, or equivalent
loading, and the loading removed. Final sag includes the effect of inelastic
deformation (creep).
2. Initial Unloaded Sag: The sag of a conductor before the application of an
external load.
G. Secondary: Conductors and components for circuits operating at the utilization voltage
of 600 V or less.
H. Service: Set of insulated conductors extending from a pole to the metering point or
service entrance connection at the location of utilization of electricity.
OVERHEAD ELECTRICAL DISTRIBUTION

PART 2 - PRODUCTS
2.1 MATERIAL
A. All material to be provided by COPA. Contractor shall provide all vehicles, equipment,
and miscellaneous tools for installation.
PART 3 - EXECUTION
3.1 GENERAL INSTALLATION REQUIREMENTS
A. Verify dimensions by field measurement, and identify locations of poles, guy anchors,
and other features. Also identify locations of connection to new and existing supply
lines and to primary and secondary services. Notify Terry Dahlquist, Electrical
Engineering Manager, 360-417-4702, of discrepancies and field conditions that are not
indicated and that will affect installation.
B. Ground all equipment on new poles according to NESC requirements. Install #6 solid
copper ground wire on each new pole, per COPA Standard H001 or H002.
C. To the extent possible, work shall be performed with conductors and system energized.
Contractor shall provide COPA with a minimum of 72 hours notice of any outages
required. COPA shall schedule outage with affected customers. Outages shall be
limited to 10 hours maximum continuous time in one day. Work shall continue in any
outage situation until service is restored to all customers.
D. Clearances originating from Light Operations Dispatch Office will be issued to City of
Port Angeles Light Operations Personnel. The Person holding this clearance will be
the responsible person who has been delegated the authority to oversee or perform
specific duties under certain conditions and in accordance with specific instructions.
Light Operations, Accident Prevention Plan 13.04 (b) 3. When a contractor's crew
is working on the City lines or equipment, the City's designated representative shall be
the person taking the clearance. The designated representative's name and radio call
number shall be included in the clearance request.
Prior to starting work each day, the contractor shall verify daily with the City's desig-
nated representative that the line is still safe and available. The line configuration will
not be changed until the City's designated representative has received written notifica-
tion from the contractor that the work is complete and that the line can return to it's
normal configuration. In the event that the configuration needs to be changed and noti-
fication cannot be obtained from the contractor, both the City's designated representa-
tive and either the Light Operations Manager or the City's Engineering Manager will
need to approve the change. Every effort will be made to inform the contractor of the
change in the status.

3.2 CONDUCTOR INSTALLATION, GENERAL
A. Handle and string conductors to prevent cuts, gouges, scratches, kinks, flattening, or
deformation. Remove damaged sections and splice conductors.
1. Conductors Reinstalled or Resagged: String to "final" sag values to maintain
clearances equal to or greater than NESC requirements under worst conditions.
B. Connections, Splices, and Terminations: Use kits listed for the specific type of
connection and combination of materials used in the connection, or recommended for
the specific use by manufacturer of material on which applied.
1. Splice Location: Do not install within 3 feet of a support.
2. Line Conductors and Service Drops: Install so strength exceeds ultimate rated
strength of conductor.
3. Splices and Terminations of Covered Conductors: Comply with manufacturer's
. written instructions.
4. Splices and Terminations of Insulated Conductors of Self-Supported, Medium-
Voltage Cable: Comply with manufacturer's written instructions.
C. On all transformers on poles being replaced, install new 2/0 copper leads from
secondary bushings down to secondary conductors or service loops. Reconnect with
compression connectors.
3.3 MEDIUM-VOLTAGE LINE CONDUCTOR INSTALLATION
A. Application: Install bare conductors, unless otherwise indicated.
B. Support line conductors and taps as follows:
1. Use preformed wire ties for conductor attachment to pin and vertical post
insulators, unless otherwise indicated. Install wire ties tight against conductor
and insulator.
2. Use wire clamps on horizontal post, dead end, and suspension insulators, unless
otherwise indicated.
3.4 POLE AND CROSSARM INSTALLATION
A. Pole Orientation: Align curve of curved wood poles with straight-line runs of three or
more poles. Align gains perpendicular to runs.
B. Elevation of Line above Grade: Install poles with top at same elevation, unless grade
changes dictate elevation change in poles, and according to the following:
1. On level ground, set poles so tops of consecutive poles vary not more than 60
inches (1500 mm) in elevation.
2. Shorten wood poles by cutting off the top,

1. Make pole holes vertical, uniform in diameter, and large enough to permit
effective use of tamping bars all around. Bore or excavate holes with an average
diameter at grade less than twice the diameter of the pole at the same grade.
2. Set poles at a depth of ten percent of the pole height, plus two feet. Minimum
depth shall not be less than five feet for any pole.
3. For poles on slopes, hole depth is measured from finished grade at lowest side of
hole.
4. Set poles in alignment and plumb except at dead ends, angles, and points of
extra strain; rake poles against conductor strain 1 inch (25 mm) minimum, 2
inches (51 mm) maximum, (after conductors are installed at required tension) for
each 10 feet (3 m) of pole length. Rake poles so they will not lean or bend in
direction of strain when loaded.
5. Backfill holes in 6-inch (150-mm) maximum lifts, and thoroughly tamp each layer
before starting the next.
6. Place surplus earth around pole in a conical shape, and tamp thoroughly to
provide drainage away from pole.
7. Set poles so alternate crossarm gains face in alternate directions, except at
terminals and dead ends; place gains on last two poles on side facing terminal or
dead end.
8. Poles Set in Concrete Paved Areas: Install poles with minimum of 6-inch- (150-
mm-) wide, unpaved gap between the pole and the edge of adjacent concrete
slab. Fill unpaved ring with pea gravel to a level 1 inch (25 mm) below top of
concrete slab.
D. Field treat factory-treated poles and crossarms as follows:
1. Poles Treated More Than One Year before Installation: Treat portion from 24
inches (600 mm) above ground line to butt.
2. Field-Bored Holes and Field-Cut Gains and Pole Tops: Treat cut portions.
3. Unused Holes: Treat and plug with treated-wood-dowel drive pins.
4. Engage the services of a technician certified according to Part 1 of this Section to
apply treatment. Comply with requirements in AWPA standards that govern
original factory treatment for field-applied treatment chemicals and application.
E. Crossarm Installation: Set line crossarms at right angle to line for straight runs and for
angles 45 degrees and more. Bisect angles less than 45 degrees.
1. Buck Arms: Install at corners and junction poles, unless otherwise indicated.
2. Double Crossarms: Install at dead ends, corners, angles, and line crossings.
3. Equipment Arms: Locate below lines and set parallel or at right angles to them,
whichever provides best climbing space.
4. Gains: Install metal pole gains only. Do not cut gains in field without specific
written approval.
3.5 HARDWARE AND ACCESSORIES INSTALLATION
A. All pole replacements include removing and installing all related pole hardware,
cutouts, transformers, etc. to match existing poles, except as noted.
B. Install washers against wood and under nuts, including eyenuts and locknuts.

C. Install nuts and locknuts wrench-tight on threaded connections.
D. Install guys to resist unbalanced loads, including those developed at angles, corners,
and dead ends. Install two or more guys if a single guy will not provide adequate
strength. Install separate guys if unbalanced loads are separated by 36 inches (900
mm) or more.
E. Protect guy strands from damage. Replace damaged guy strands. Ground guys per
NESC requirements. Install guy insulators where required to comply with IEEE C2
clearance requirements and elsewhere, where indicated. Install strain insulators to
prevent corrosion of anchors from circulating currents.
. F. Where pole and span guys connect to opposite sides of the same pole, connect to the
same pole hardware such that the tension load is on the hardware rather than the pole
wood.
G. . Guy Markers: Install at anchor end of guys. Clamp to guy strand or anchor at top and
bottom of marker.
H. Install plastic rain cap on top of each wood pole. Fasten in place with nails or lag
screws.
I. Install tag showing pole number on each new pole installed on this contract. Use
numbers and tags detailed on staking sheets.
J. Screw-in anchors shall be installed to manufacturer's recommended torque. Install
extension rods where required to achieve proper installation.
3.6 INSULATOR INSTALLATION
A. Medium-Voltage Line Application: Install pin type, except install suspension type at
corners, angles, dead ends, and other locations where horizontal forces exceed rated
values for pin or line-post-type units.
1. Install suspension insulators and hardware that have mechanical strength
exceeding rated breaking strength of attached conductors.
2. Install horizontal line-post insulators for armless construction.
B. Post-Insulator Conductor Support: Where installed horizontally and for line angles
more than 15 degrees, install clamp-top conductor clamps.
C. Install spool-type insulators for secondary lines mounted on clevis attachments or
secondary racks.
D. Guy Strain Type: Install fiberglass-reinforced units where specified.
3.7 CUTOUT, SWITCH, AND FUSE INSTAllATION
A. Hook-Stick-Operated Switches: Install to maximize safe operating access.

B. Group-Operated, Load-Interrupter Switches and Air-Break Switches: Install operating
handle 42 inches (1067 mm) above finished grade.
1. Locking Provisions: Install padlock at hasp.
3.8 FIELD QUALITY CONTROL
A. COPA representative will monitor and inspect final equipment installation. Any
installation deficiencies identified will be corrected by the Contractor at no additional
cost to the City.
B. Contractor shall perform the following tests and inspections and prepare test reports:
1. Furnish instruments and equipment required for tests that comply with
NETA ATS.
2. Measure resistance of each separate grounding electrode, including pole
grounds. Also measure resistance of separate grounding electrode systems
before bonding together.
a. Perform tests and obtain acceptable results before energizing any portion
of overhead electrical distribution system.
b. Results and Follow-up: If ground resistance for a single ground electrode
or pole ground, tested individually, exceeds 25 ohms, add a ground
electrode not less than 10 feet (3 m) away and interconnect with NO.2
AWG minimum bare conductor buried at least 12 inches (300 mm) below
furnished grade.
3. Verify that initial stringing sags and tensions comply with IEEE C2 and conductor
manufacturer's Product Data and written recommendations.
4. Neutral-Supported, Secondary Service Drop Cable: Test for insulation
resistance while cable is isolated, before connecting or energizing. Minimum
acceptable resistance is 100 megohms.
5. Existing Surge Arresters: Disconnect and measure resistance between line and
ground terminals with a megger rated 600 V or more. Acceptable resistance
values are 300 megohms and more.
6. New Surge Arresters, Cutouts, and Switches: Inspect after installation and
connection to wiring. Verify that ratings and characteristics match approved
submittals and comply with system requirements. Verify that installation
complies with requirements and that clearances of units and connecting wiring
comply with IEEE C2 requirements.
a. Verify proper grounding of metallic equipment parts.
b. Fuses and Disconnect Links: Verify that ratings and characteristics match
submittals and comply with system requirements.

SUBJECT: Issuing of electrical clearances to non-city employees

Jim Klarr, Light Operations Division
Clearances originating from Light Operations Dispatch Office will be issued to City of
Port Angeles Light Operations Personnel. The Person holding this clearance will be the
responsible person who has been delegated the authority to oversee or perform specific
duties under certain conditions and in accordance with specific instructions.
Light Operations, Accident Prevention Plan 13.04 (b) 3. When a contractor's crew is
working on the City lines or equipment, the City's designated representative shall be the
person taking the clearance. The designated representative's name and radio call number
shall be included in the clearance request.
Prior to starting work each day, the contractor shall verify daily with the City's designated
representative that the line is still safe and available. The line configuration will not be
changed until the City's designated representative has received written notification from
the contractor that the work is complete and that the line can return to it's normal
configuration. In the event that the configuration needs to be changed and notification
cannot be obtained from the contractor, both the City's designated representative and
either the Light Operations Manager or the City's Engineering Manager will need to
approve the change. Every effort will be made to inform the contractor of the change in
the status.

Clarification and Additional Construction Standard Sheet:
1) Re: 10286 - The term "St U" refers to there only being a street light on that pole, \Mth an overhead feed.
The closest standard is S004-A, as noted in the "Mise" column. That shows the light mounting; there is no
primary above it.
2) Re: 10352 - Standard PF105 was not included in the Bid Documents. It is attached to this addendum to
become part of Appendix B - Construction Standards.
3) Re: 9487 A - Contractor must coordinate any closures of Tumwater Truck Rt. with Washington
Department of Transportation (DOT), the City of Port Angeles, and Parsons, Inc., the contractor on the 8th
Street bridges replacement. DOT typically requires 3 to 4 weeks advance notice. Any closure shall be done
to minimize impact on the aforementioned entities. No additional costs \Mil be allowed by the City resulting
from such closure for this contract or to its construction contract on the 8th Street Bridges replacement.
