Dear Mr. Faulk:
As requested, we have determined the elevation for roof height of your home at the
property listed above. .
An average ground elevation at the perimeter walls was determined on March 23,2007 to
be an assumed elevation of 98.35 feet, which would allow you to construct the proposed
building to a height not exceeding 128.35 feet.
On April 28, 2007, we returned to the site to measure the ridge elevation. This was
determined to be at an assumed elevation of 127.18 feet. Therefore, t!Ie building was
constructed at 1.17 feet below the maximum height allowed.

9. Waste Analysis:
The "Dangerous Waste Regulations" (WAC 173-303) shall be utilized to determine the
appropriate analytical requirements for waste characterization. Ecology Publication #91-30
(Revised April 1994) "Guidance for Remediation of Petroleum Contaminated Soils" shall also be
used to characterize petroleum contaminated soils from UST releases. Submit all laboratory
analytical results, QAlQC data, and Chain of Custody sheets along with this application.

Scope of work
1) Good faith inspection for asbestos containing building materials (ACBM).
2) Survey, sample and record suspect materials.
3) Report to owner William Faulk.
Inspection Reoort
The inspection started with a visual survey looking for Asbestos Containing Building
Material (ACBM).
A single family residents built in the early 30's. Approx. 900 sq. ft.
Built on concrete foundation, wood siding, composition roof, electric heat, all carpeted
floors over wood and vinyl clad windows.
No suspect of ACBM.
Summary of Inspection
All asbestos containing building materials with a reading greater than 1% is
considered a hazardous material if disturbed.
If removed the owner or a certified abatement contractor must follow the rules of the
EPA and governed by Olympic Region Clean Air Agency.
During demolition it is possible that additional suspect asbestos containing
building material (ACBM) may be found. Should such suspect material be
discovered an AHERA certified inspector will have to sample and test the
material to prove it is of non-asbestos.
Northwest Asbestos Consultants is not responsible for identification of hidden
materials that are not identifiable with reasonable diligence.
