PROFESSIONAL SERVICES AGREEMENT
FOR
THE CITY OF PORT ANGELES

Scope of Services:
Polaris Engineering and Surveying, Inc. will provide professional services. in
conjunction with the site preparation and building construction for the Port
Angeles Public Library, to be constructed at the southwest corner of East
Lauridsen Boulevard and Peabody Street in Port Angeles. Services will include
the follpwing:
Observation of the placement of structural fill and compaction testing of
structural fill material as set forth in the Contract Documents for the project

Compaction testing of subbase, base, and top rock material for the project

Observation of concrete placement and testing of the concrete as set forth
in the Contract Documents
Observation of asphalt concrete placement and compaction testing as set
forth in the Contract Documents
Coordination of testing of structural steel if requested
Polaris will retain PSI (formerly Pacific Testing Laboratories) to provide laboratory
analysis of materials as required and for structural steel inspection.
Professional Fees:
Polaris will charge on a Time and Materials basis for services rendered and costs
incurred according to the attached List of Charges for Professional Services.
Billings will be on a monthly basis. Payment is due upon receipt of the bill and is
considered delinquent 30 days from the billing date. Polaris assesses a 1 % per
month service charge on delinquent accounts. Polaris reserves the right to
suspend work on projects for which payment is delinquent.
Polaris offers a 2% discount for payments received within 10 days of the billing
date.
Time:
Polaris will provide on-site inspections within 24 hours of requests to do so by the
Contractor.
Authorization:
Polaris Engineering and Surveying, Inc. is authorized to proceed with the work on
this project subject to the terms and conditions set forth herein. The Client
agrees to pay reas a a' r 's fees for collection of delinquent payments for

Submittal!!: Submit the following:
Product data for reinforcement, forming accessories, admixtures, patching compounds,
waterstops, joint systems, curing compounds, and others as requested by Architect
Shqp drawin~ for fabricating, bending, and placing concrete reinforcement.
l.ahnrauuy test reports or evaluation reports for concrete materials and concrete mix designs.
Written report to Architect for each proposed concrete mix at least 15 days prior to start of
concreting. Do not begin concrete production until mixes have been reviewed by Architect
Duality Assurance: Comp-Iy with provisions of ACI 301, "Specifications for Structural Concrete for
Buildings," ACI 318, "Building Code Requirements for Reinforced Concrete," and CRSI "Manual of
Standard Practice," except where more stringent requirements are indicated.

straight, smooth, exposed surfaces.

Concrete Materials: As follows:
Portland Cement: ASTM C 150, Type 1.
