RE: Sanitary sewer extension

This letter is in regards to a recent connection to the existing sanitary sewer main in Del Guzzi
i ~ Drive. An eight inch private sanitary sewer gravity main was installed by Crescent
Development per the approved utility plan submitted to the City of Port Angeles, Public
Works & Utilities, Engineering Division by Zenovic and Associates for James and Roberta
Mantooth.

All improvements have been inspected, tested, completed and approved by Public Works and
Utilities. Upon payment of the applicable fees, the building sewer lines may be connected to
the sanitary sewer main. Each building sewer line is to be inspected by the Public Works &
Utility inspector prior to backfilling the trench. Existing building septic tanks shall be
pumped, crushed and or backfilled per Clallam County Environmental Health Department
requirements. Copies of any permits issued or inspection sheets noted on decommissioned
septic tanks should be sent to the City's for our building record keeping.
