permit. It has taken longer than anticipated to secure our necessary materials to start this project. We
would like to request an additional 180 day extension for the existing permit. If this is acceptable please

copies. Keep one for the contractor, which will need to be posted on site dunng
construction. Please return the other signed copy to our office.
I have included two electncal permit applications which are separate from the building
permit. These will need to be applied for at the time the work is started. Any questions

Per conversation on 9/14/05 with Pat in the Building Department, attached is the required
fee for issuance of the Building Permit for the Venzon Wireless project to add antennas
to and existing Bonneville Power Authority tower located at 2807 Porter Street. Per Pat's
direction, the check is enclosed to issue the permit. If possible please sign the attached
Confirmation of receipt and use the enclosed UPS overnight package to mail the permit
to our Kirkland, WA office. I appreciate working with the City of Port Angeles on this
development and look forward to possible future projects.

VALUATION OF CONSTRUCTION In all cases, a valuation amount must be entered by the applicant. This figure will be reviewed
and may be revised by the Building Division to comply with current fee schedules. Contact the Permit Coordinator at 417 -4815 for assistance.
PLAN CHECK FEE IF a plan check fee is due it must be submitted at the time the building permit application and construction plans are
submitted. All other permit fees are due at the time of permit issuance.
EXPIRATION OF PLAN REVIEW If no permit is issued within 180 days of the date of application, the application will expire. The
Building Official can extend the time for action by the applicant up to 180 days upon written request by the applicant (see Section R105.3 2
of the International Building /Residential Code, 2003) No application can be extended more than once.

Dear Mr Renken.
As you know, following a public heanng conducted on June 8, 2005,. the Planning
Commission approved the co- location of a telecommunications facility at 2807 Porter Street
as described in your application to this department. The facility will be co- located on an
existing tower within the Bonneville Power Substation site per the following conditions
Conditions.
1 The applicant shall construct the additional antennae array on the existing utility pole
located at 2807 Porter Street, Port Angeles, per the Conditional Use Permit
application submitted on May 2, 2005, to the City of Port Angeles. All standards for
the proposed construction shall be observed and as part of the development,
Bonneville Power Association will assess and venfy the structural adequacy of the
structure
2 The support site will require .a building permit issued by the City of Port Angeles.
Utility service shall be underground at the applicant's expense
