Enclosed is Surface Mining Reclamation Permit Number 70- 012560, which authorizes continued
surface mining reclamation within the 25 -acre portion of the southwestern quarter of the
northwestern quarter, Section 17, Township 30 North and Range 6 West, Willamette Meridian,
Clallam County Please refer to the permit number listed above when submitting inquiries or
reports
RCW 78 44 requires that reclamation of each segment of the permitted area shall be completed
within two years of cessation of mining in that segment. We strongly recommend, however, that
reclamation of each segment occur concurrent with the removal of the minerals

Pursuant to RCW 78 44 a Reclamation Permit hereby granted to the above -named permit holder to engage in surface mining
on the property described in the application and material on file under this permit. The total area to be disturbed by surface
mining, including the deposition of surface mining refuse, shall be in accordance with the reclamation plan filed with and
approved the by the Department of Natural Resources under this permit, and in accordance with the conditions and
descriptions set forth in Exhibit A attached hereto and made a part hereof, and RCW 78 44
TERM OF PERMIT
This permit shall be in effect from the date of issuance and shall remain in effect so long thereafter as the permit holder pays
the annual basic fee for each site, complies with the Surface Mining Act and the rules promulgated thereunder complies with
the reclamation plan, and maintains a performance security as required by the Act.
CHANGE OR MODIFICATION OF RECLAMATION PLAN
The permit holder shall obtain written approval from the Department prior to any change or departure from the approved
reclamation plan.
PERFORMANCE SECURITY
A performance security shall be submitted to and approved by the Department prior to commencement of surface mining. The
permit holder may submit a cash deposit, assignment of a savings account or certificate of deposit, bank letter of credit,
negotiable securities, or a corporate surety bond in the amount specified. The amount of the performance security shall be
subject to adjustment according to RCW 78.44
TRANSFER OF PERMIT
The transfer of this permit to another permit holder shall not be made unless approved in writing by the Department. A transfer
shall not be approved unless the successor permit holder assumes all duties of the former permit holder to complete the
reclamation of the land and the Department approves the successor permit holder's performance security
PENALTIES
The Department may suspend surface mining or impose civil penalties if the permit holder conducts surface mining in any
manner not authorized by RCW 78 44 the rules adopted thereunder the approved reclamation plan, or this permit.

1 This surface mining reclamation permit applies to the following permit area. a 25 -acre
permitted area in a portion of the southwestern quarter of the northwestern quarter, of
Section 17, Township 30 North, Range 6 West, Willamette Meridian, Clallam County

2 All mining and reclamation shall comply with the rules and regulations pursuant to Chapter
78 44 RCW and Chapter 332 -18 WAC
3 All mining and reclamation shall comply with the reclamation plan dated March 24, 2011 by
the permittee and approved by the Department of Natural Resources on August 02, 2011
Any amendment or change to the plan must be submitted to and approved in writing by the

approved reclamation permit (which includes the reclamation plan, maps, and supporting

metal stakes and survey markers as described in Section 23A of the application SM-
8A. The metal stakes and survey markers will be used to delineate the permit boundary
enabling the operator, permittee, and the Department of Natural Resources (DNR) to
monitor the progress of mining and reclamation. Permanent boundary corners shall be
visible such that the entire perimeter of the site could be reestablished using the Global
Positioning System, if any marker was destroyed. All permit area fences shall be
maintained for the duration of this reclamation permit.
6 The permitted depth at this site is 25 vertical feet from the original ground surface or
interception of ground water Should ground water be intercepted, mining will cease and an
evaluation will be conducted on the affects of the mining on adjacent wetlands and aquifers
No excavation shall occur below an elevation of 530 -feet mean sea level (msl) If additional
mining depth is anticipated the reclamation plan shall be revised prior to exceeding the
permitted depth.
7 A minimum of 35 feet for "screening setback" was required by Clallam County Conditional
Use permit No 87 -0007 The screening setback is outside of the permitted boundary
Therefore, no additional reclamation setback is required. The Department does not
authorize access of adjacent properties to perform any reclamation activities
8 Based upon existing conditions, topsoil was previously stripped and stockpiled in berms on
the east, south, and west perimeters of the site Topsoil is required for reclamation.
Approximately 6,000cy of topsoil is stored on the site to provide the minimum cover on
reclamation slopes and the pit floor The pit floor will only receive a minimum amount of
topsoil due to the future industrial and residential development plans.
9 Reclaimed slopes shall vary in slope angle but be no steeper than 2H 1V and be sinuous in
plan and profile The mine floor will be reclaimed to a relatively flat grade and reseeded to
dry land grasses if not re- developed within one year of final site reclamation. Reclamation
design considerations have been established to support future industrial and residential site
development.
10 Vegetation will be reestablished on the mine floor and slopes as per the conditions set forth
in the SM -8A and Narrative dated permittee on March 24 2011, and February 1 2010
respectively Revegetation shall commence during the first proper growing season after
slope creation and topsoil distribution. Noxious weeds shall not be considered acceptable
vegetation. Only pioneer species shall be deemed satisfactory revegetation.
11 The permit holder or operator shall maintain a complete copy of this permit, including these
conditions and the approved reclamation plan with additions and amendments, at the mine
site during all mining and reclamation activities.
12 Final land use after reclamation for this mine is industrial and residential development. The
department is not responsible for preparing the site for this future development. Rules and
regulations according to the local jurisdictions will control the site's future industrial and
residential land use

ensure that all best management practices are employed and that water quality is protected.

activities. Mining and reclamation must be considered together as closely related activities and
not as two separate functions. All mmmg must be planned with the reclamation goals in mind and
all reclamation activities must be planned with the most efficient mmmg sequence m mind. This
will not only maximize Lakeside's utilization of the aggregate resource, but will prevent the
unnecessary moving back and forth of soil stockpiles and berms. Furthermore, it allows
sequential reclamation to take place m the off season when personnel and equipment may
otherwise be under utilized.
General.

b This plan will be kept on file with the Environmental Program Director, with the
Division Manager, with the DNR Inspector, and with the Crushing /Mining Operator
Commumcation is important between everyone so that mining and reclamation are both
concurrent and efficient. Any questions about this plan must be addressed to the Environmental
Program Director
c. This document is only a plan. Minor changes should be expected due to aggregate
quality, new equipment, market conditions, and /or other factors. Any proposed changes to the
plan by Lakeside will be communicated to the DNR through the Environmental Program Director
If necessary a revised reclamation plan can be submitted.
Responsibilities.
a. Lakeside Environmental Program Director This person keeps current the
reclamation plan m comphance with DNR guidance and regulations. This must be done in close
concert with the Division Manager The Environmental Program Director will also conduct
periodic inspections to ensure the steps and goals of this plan are being carried out.
b Lakeside Division Manager This person will implement the mining and reclamation
plan with regard to all mining and reclamation activities. The division Manager is the
communication lank to the bucket loader operator on site during normal operations throughout the
year He or she will be responsible for all removal and placement activities at the mine.
c. Mining/Crusher or Pit Operator The operator on site is responsible to conduct
mining and reclamation activities in support of this plan. He or she must have a clear
understanding of the reclamation goals as well as the limits of mining with regard to boundaries
and phase lines. In general this should be done by scheduling an on -site discussion with the

Narrative Plan.
This site was ongmally leased by Lakeside from the Weyerhauser Company as a gravel
source m the 80s. The property was then purchased by Lakeside Industnes. Clallam County
issued a Conditional Use Permit in 1988 to Lakeside to continue mining after an Environmental
Checklist was determined to have insignificant effects. Because the DNR permit maps are
outdated, this narrative and the attached maps will clanfy Lakeside's efforts to mine and reclaim
the pit in a responsible manner
The property is located just south of Lakeside's Port Angeles Division office. The
neighboring businesses to the north and west are of the logging /wood products nature, and to the
south and east are residential areas. The majority of the site itself is located within the city limits
of Port Angeles and is zoned Industrial. There is a small parcel located in the southeast corner of
the site that is under Clallam County junsdiction. This parcel is zoned residential by Clallam
County
The current site topography shows that the majonty of the mining has occurred in the
northern -most area of the permitted boundary This area has since been sloped and is now used as
a storage area for sand and gravel. In the central area of the site, some matenal has been taken but
not to full depth. The southern area of the site can be divided into three sections. The southeast
and the southwest will still be mined, and the south central area will not due to the quality of
material.
The property includes the mine area, a small structure for storage on the east side of the
property, and the northern area which has been leased as log storage to another company This
north end of the property will never be mined. No other structures or scales are present on the
site. No permanent crushers or batch plants exist. All aggregate sorting and crushing activities are
performed with mobile equipment.
Topsoil has been saved on this site, the majonty having been pushed into berms along the
east, west and south property boundaries. Due to the nature of drainage on the site, all
stormwater infiltrates and there are' no surface water discharges. This water tends to pond m
either the east or south ends of the site.
As a condition of the Conditional Use Permit, a 35 foot setback for sight screening has
been established. This is reflected in the attached maps.
In general, the mining can proceed in three phases, all of which to an approximate depth of
25 feet below the original ground surface. Phase 1 will consist of mining in the southwestern area
of the site. Topsoil has already been moved to the west side of the site between the mining area

and the road. The Phase 1 area will be mined by the cut -to -slope method with a rubber -tired
front -end wheel loader After the sand and gravel has been extracted, the edges of Phase 1 should
be cleaned up to ensure proper 2 1 slopes and create variability After the Phase 1 area is
completed, it will be seeded.
Phase 2 is located in the southeast corner of the site. To first mine in the Phase 2 area, the
storage building will have to be moved or demolished. Lakeside will seek the applicable permit to
handle this operation at the appropriate time. The mining will be conducted using the same
equipment and technique as was used in Phase 1 Upon completion of milling, the Phase 2 area
will be cleaned up to ensure proper 2 1 slopes and create vanabihty, and then seeded.
Finally, Phase 3 compnses of the area in the center of the site. This area has already been
stripped of topsoil, but has yet to be mined to full depth because it has been used as a pad for
mining equipment and aggregate storage. When the first two phases are completed, phase 3 can
be mined to a depth approximately 20ft below the current ground surface. No sloping will be
required in phase 3 as it is located m the center of the site.
Final Reclamation.
After mining, processing, and stockpile activities have ceased, the site will be prepared for
final reclamation. All mine equipment and related materials will be removed from the property
The edges of the mine that were previously reclaimed at the end of each phase will be addressed
agam if there were any issues with slope, variability, or growth.
The subsequent use approved for the majority of the site is Light Industrial/Log Yard, and
the southeast corner is approved as residential. The Industrial use is compatible with Lakeside's
ongoing operations, the neighboring businesses m the area, and the current tenant who leases the
northern edge of the property Because of this use, the best way to reclaim the mined areas are to
slope and seed the edges of the property to a 2 1 or flatter slope, and grade the mine floor to flat
workable surface that meets the needs of the Industnal Use. The southeast corner (residential
use) will be seeded and sloped.
If the site is not used, leased, or sold immediately, Lakeside will protect the mine floor
from erosion. The mine floor will be ripped with a dozer to break up any hard compacted surfaces
such as haul roads. The floor of the mme will then be topsoiled and vegetated. Existing storage
piles on the edges of the mine will be utih7ed. Lakeside will seed the site with grasses and clovers.
Typically used amounts are approximately 7lbs /acre of seed placed by hydro mulching with
tackifier, although we will consult with the seed supplier for the recommended apphcation rate.
