Are you installing a mop sink. If so does it have an
atomspheric vacuum breaker on the phauset? Does the heat
pump have a water supplied going to it? If it does, then it
needs a backflow assembly.

The General Archaeological Permit Conditions are as follows:
1. The applicant must hire an approved archaeologist to be
present on site during all ground disturbances. The project
proponent will be required to provide the City with the name
and credentials for their archaeological representative(s)
as well as a brief work plan and schedule ahead of the
permitted ground disturbance. The Archaeologist must meet
the Washington State guidelines for a Professional
Archaeologist and must be in contact with the City
Archaeologist prior to and during the permitted project. A
professional monitoring report must be issued to the City
Archaeologist upon completion of the ground disturbing
actions.
2. Regardless of the presence of an archaeological monitor,
if during ground disturbances,' any phenomena of possible
archaeological interest are uncovered the contractor shall
immediately stop such work and provide for a site inspection
and evaluation by the City Archaeologist to ensure that all
possible archaeological resources are handled in accordance
with applicable law. In the event archaeological artifacts,
features or human remains are discovered, the City will
notify the Lower Elwha Klallam Tribe and the State
Department of Archaeology and Historic Preservation, as
required in RCW 27.44 and 27.53. Archaeological Preservation
requirements will then be enacted as applicable to State
law.

The proposal will result in a 3119 sq.ft. addition to an
existing office in the IH zone outside of the shoreline
environment for a total of 7% lot coverage and 9% site
coverage. No additional parking will be added as a result
of the addition so no SEPA review is required.

The following are changes for the project:
1. The 2 -10 door on the second floor level at the firewall between the new and existing is
being changed to 3 -0 due to availability. We are still considering it not to be a required
egress door.
2. We have elected not to install dead bolts on the exterior doors, thus not installing the
signs regarding open during business hours.
3. We are showing changes to the framing (2 x 8 studs) at the south bathroom wall for the
plumbing.
4. As an alternate for the vertical bars under the vents hooked at the top to act as stirrups
for shear reinforcement to transfer hold down forces per detail 2/14, some X bars with
hooks as per calculation sheet 19 and special inspection report #2 are installed.
5. The 2 x 12 floor joists are being replaced by Roseboro Forest Products Timber I
joists 11 7/8" RFPI 40s. The 2 x 8s may be replaced with 9'/4" RFPI 40s.

Provide dowel extensions to lap with rebar at adjacent "monolithic" footings that
will be placed later. Add vertical dowels with hooks at edge of openings (vents)
and hold downs. Remove loose soil and rocks within 3" of rebar.
ALLOWED MODIFICATIONS TO PLANS: On plans, change top rebar on vent
reinforcement detail to (2) #4 from (2) #5. On type 2 and 4 hold downs, #3
dowels within 2' /i" of hold down bolts hooked into footing may be replaced with
#4 dowels within 4

OBSERVATIONS: Longitudinal and vertical reinforcement as vertical dowels
extended from footing or as lapping vertical dowels extended from footing. Hold
down bolts and anchor bolts in place. Hooks at top of vertical bars acting as
stirrups under vents were missing at short stemwalls where there is insufficient
distance for lap splicing.
REQUIRED MODIFICATIONS: Provide 45 °diagonal bars with 4" horizontal
hooks each end per detail under vents at vertical bars missing top hooks under
vents. Provide dowel extensions from the stemwalls to lap with rebar at adjacent
"monolithic" footings that will be placed later.
ALLOWED MODIFICATIONS TO PLANS: Move the 7/8" o anchorage bolt for the
8Q holddown for the corner at the north end of the east wall to the east end of
the north wall where it can lap with 2 #4 vertical rebar that are hooked into the
footing. The rner studs attached by the holddown will need to have the
plywood fro both walls edge nailed to them and the multiple studs attached to
each other p r the holddown notes. W o anchor bolts may be threaded rods (to
allow the 4" hreaded extension above the plate) with a slight bend in the 6"
embedme into the stemwall.

OBSERVATIONS: Some holes were drilled, wire brushed, and cleaned by
oilless air pressure. Cleanliness and depth were checked on these as well as the
new holes observed to go through the same process. About half the holes
required additional wire brushing and blowing out. Simpson Epoxy Set XP
expiring September 31, 2012 was properly inserted in the holes and the rebar
inserted, twisted and brought out prior to adding more epoxy reinserting the
epoxy covered bar, more twisting and air bubbles popping at the surface.
ALLOWED MODIFICATIONS TO PLANS: On plans, change rebar to go through
embedment to 5" from 6
ADDITIONAL COMMENTS: Full time special inspection by Craig Owen and /or
Bill Dawson under my direction.

The General Archaeological Permit Conditions are as follows:
1. The applicant must hire an approved archaeologist to be present on site during all ground
disturbances. The project proponent will be required to provide the City with the name and
credentials for their archaeological representative(s) as well as a brief work plan and
schedule ahead of the permitted ground disturbance. The Archaeologist must meet the
Washington State guidelines for a Professional Archaeologist and must be in contact with the
City Archaeologist prior to and during the permitted project. A professional monitoring
report must be issued to the City Archaeologist upon completion of the ground disturbing
actions.
2. Regardless of the presence of an archaeological monitor, if during ground disturbances,
any phenomena of possible archaeological interest are uncovered the contractor shall
immediately stop such work and provide for a site inspection and evaluation by the City
Archaeologist to ensure that all possible archaeological resources are handled in accordance
with applicable law. In the event archaeological artifacts, features or human remains are
discovered, the City will notify the Lower Elwha Klallam Tribe and the State Department of
Archaeology and Historic Preservation, as required in RCW 27.44 and 27.53. Archaeological
Preservation requirements will then be enacted as applicable to State law.
