LANDFILL CELL STABILIZATION,PROJECT NO.02-12 RELATING TO: Landfill Cell Stabilization Project Construction Management Services THIS AMENDMENT No. I is made and entered into this 2--5, day of WOW, 2015, by and between the City of Port Angeles, a non-charter code city of the State of Washington, (hereinafter called the "CITY") and Anchor QEA, LLC. a Washington Corporation authorized to do business in the State of Washington (hereinafter called the "CONSULTANT"). NOW, THEREFORE, in consideration of the representations and the terms, conditions, covenants and agreements set forth in the original AGREEMENT and this AMENDMENT,the parties hereto agree as follows: SECTION 1 The City entered into the existing AGREEMENT with CONSULTANT on April 15,2014, (the AGREEMENT). SECTION 2 I. AMENDMENT SCOPE OF WORK The scope of professional services to be performed and results to be achieved by the CONSULTANT is amended to include additional work as described in the attached amended Exhibit A. The CONSULTANT shall provide and furnish all services and material necessary to accomplish the additional work identified in amended Exhibit A. IV. TIME OF PERFORMANCE All Tasks shall be completed in accordance with the schedule set forth in the attached Exhibit 134. VI. MAXIMUM COMPENSATION �3 The CONSULTANT'S total compensation and reimbursement is amended to$1,648,927.34. The maximum compensation set forth in the attached Exhibit C is$219,927.34. The following exhibits are hereby made part of this Amendment: Exhibit A - Scope of Services of Tasks Exhibit B - Schedule of Performance to Tasks Exhibit C - Budget of Tasks Except as modified herein,the original AGREEMENT is confirmed and ratified. Bloor, City Attorney PALF Cell Stabilization CM Contract Amendment 1,Page 2 November 19,2015 EXHIBIT A SCOPE OF SERVICES OF TASK 1 Additional taskl, described below,represent the scope of work required to provide quality assurance testing related to the low permeable soil barrier placement on the 351 and 304 Cells as well as the MIZE (mechanically reinforced earthen)wall. BACKGROUND The original Scope of Services dated April 15, 2014 provided for the CONSULTANT to provide construction management,consultation, and field observation for the removal of refuse from the eastern portion of the 304 Landfill and relocation to the 351 Landfill located further inland to the south,reconfiguration of the 304 Cell to meet geotechnical, seismic stability,and solid waste landfill closure standards. Stabilizing the seawall ends to protect the existing structure function,to accommodate the continuing shoreline retreat over time, and minimizing environmental impacts. Improving the Dry Creek/Shoreline interface to address overtopping events,reduce the destabilization risk at the west-end, add in stream roughness elements to help maintain the thalweg separation from the access road, and provide access for beach nourishment. Task 1 was added as part of Change Order 10, the Contractor's scope of work was reduced and additional scope was undertaken by Anchor QEA for the required materials testing for the low permeability soil barrier. The specific tasks involved for Amendment 1 are: TASK 1 —TESTING OF LOW PERMEABILITY SOIL BARRIER Objective: To perform the required material testing for low permeability soil barrier under Contract Specification Section 3123 02,Paragraph 1.06. Activities: Consultant shall provide the following engineering services: A. Complete all required material testing for low permeability soil barrier using a mobile testing firm. B. Monitor low permeability soil barrier pug mill operations and placement procedures to verify the materials and construction methods are in compliance with the technical specifications. C. Provide documentation of all tests and test results for low permeability soil barrier placement. D.Provide management and oversight of the mobile testing firm.
