MUTUAL SETTLEMENT AND RELEASE OF CLAIM

for utility account underpayment, do hereby acknowledge the full and complete settlement of any
claim or demand of any nature whatsoever that I, my heirs, successors or assigns, have or could
have against the City of Port Angeles, or any of their respective officers or employees, arising
from or in any way related to the billing of my utility account prior to the date of my signature.
