Change Title:
Change Order # 1 — Street Pavement Repair at E Street
Change Order # 2 — 10" Street and Park Ave. Pavement Repair
Justification:
Additional pavement damage has been identified on 8`h Street
near E Street, 10`h Street between N and M, and Park Avenue near
Peabody Street. Adding this work to the current contract will
result in project savings compared to going back out with a
separate small contract for any of the individual locations.
Change Scope:
Contractor will perform pavement repair as outlined in Contract
for the additional locations:
Change Order #1 — Work includes approximately 2,100 square
feet of arterial pavement repair as marked out by the engineer on
8' Street. A lump sum amount of$14,000 has been agreed upon.
Change Order #2 — Work includes approximately 11,702 square
feet of arterial pavement repair as marked out by the engineer on
10`' Street and Park Avenue for a lump sum of$86,000.

Change Scope/Justification:
Additional pavement damage has been discovered on 8th Street near E Street. Contractor will perform
pavement repair as outlined in contract at this additional location. This work includes approximately

open at all time, City will provide traffic control. A lump sum amount of$14,000 has been agreed upon.
