AMENDMENT NO, 1 TO MASTER SERVICES AGREEMENT
This AMENDMENT No. 1 ("Amendment")amends that certain Master Services Agreement, dated Aprd 1,
2015 (the "Agreement'), by and between City of Port Angeles, Washington, and CLEAResult Consulting Inc., a
Texas corporation.
AMENDMENT
I This Amendment is effective as of November 28, 2017 (the "Effective Date"),
2. The Agreement is amended as follows'.
a. In Exhibit A(Statement of Work No, 1),the Completion Date in Section 4(Time of Performance for
the Services) is replaced by January 31, 2019.
b. In Exhibit A-1 (Amendment to Exhibit A No, 2), the Completion Date in Section 4 (Time of
Performance for the Services) is replaced by January 31, 2019,
c. In Exhibit A-2 (Amendment to Exhibit A No. 3), the Completion Date in Section 6 (Time of
Performance for the Services) is replaced by January 31, 2019.
d, In Exhibit A-3(Statement of Work No. 4), "January 31, 2018" in Section 3 (Schedule) is deleted and
replaced by January 31, 2019.
e. Attachment A-1 is deleted in its entirety and replaced with Attachment A-1, attached and incorporated
into this Amendment,
f The funding arnount in Exhibit A-1 Section 2(b)is increased by$30,000,00 to$122,000.00
g, The funding amount in Exhibit A-2 section 2(b) is increased by$2250.00 to$5,750.00
3, All capitalized terms not otherwise defined in this Amendment shall be defined as indicated in the
Agreement. All other terms of the Agreement shall continue in full force and effect.
The parties to the Agreement have caused this Amendment to be duty executed on their respective
behalf as of the date above.
