SINGLE AGENCY TRAINING AGREEMENT
This agreement between the Port Angeles. City of and the Department of Personnel Organi~tional & Employee
Developm~nt S~rvices (OEDS) is established in accordance with the conditions specified herein. This agreement is
-'entered il1to pursuant to the Interagency Reimbursement Agreement between the parties. and is subject to all terms of
that agreement which are not inconsistent with the terms of this agreement. .
1. COURSE OR SERVICE INFORMATION - See Attachment #1.
2. COURSE DESCRJPTON(S) & OBJECTIVES - See Attachment #2 if applicable.
3. SCOPE OF WORK - See Attachment #3 if applicable.

6. AGENCY RESPONSIBILITIES
a. Identification of areas for special emphasis.
b. Designation and registration for participants on-line through HRDIS prior to the course or activity.
c. Notification of participants if course confirmation notices are not provided by OEDS per Par,agraph 7d.
d. Distribution Of pre-course mate('ials to parti,cipants a,s required. " .' ,,' '::'::'
e. Notification to oEDS If persons of disability are enrolled that need -special accommodations.:' <~; __,',
f. Payment of course or other training service cost to the Department of Personnel upon'recelpU;f billing.

h. A completed Single Agency Service Evaluation form for each instructor associated with this entire -agreement. '
If this agreement spans several months of instructional services for one or more instructors, forms may be
returned after all services are completed. . -
7. OEDS RESPONSIBILITIES
a. Staff assistance/support to the agency as required in the development, coordination, anc;f delivery of t~e
course or other service activity.
b. Assistance in identifying/selecting qualified instructors/consultants.
c. Contract and payment administration.
d. Confirmation notices when participants are pre-registered a minimum of 12 working days prior to the start of
the course or service activity.
e. Course rosters from HRDIS when participants are_pre-registered.
f. HRDIS data entry to individual profiles of participants.
g. Provide accommodation if persons of disability are enrolled that need special accommodations.

Agency Provides:
Computer training lab with software, licenses, set-up
