this permit and understand that it is my responsibility to determine what permits are required and to
obtain permits prior to work. I understand that plan review fees are not refundable after review has
occurred. I understand that I will forfeit review fees if I withdraw the application before the permit is
issued. I understand that if the permit is not picked up/issued within 18o days of submittal,the application
will be considered abandoned and the fees will be forfeited.

"General Requirements"and"Roof Deck Requirements"are part of these specifications.
1. Mechanically attach with approved fasteners an approved insulation to an acceptable deck.
2. Up to 1 Oft.wide reinforced SR TPO sheets are mechanically fastened along the seams with approved
fasteners and plates at the approved spacing.
3. Up to 5 ft.wide perimeter sheets are mechanically fastened along the seams with approved fasteners and
plates at the approved spacing.The wind zone, height of the building, parapet,and dimensions of the roof
area will determine the number of perimeter sheets
4. Adjoining sheets are overlapped and joined together with a minimum of 1 1/2 in wide hot air weld.
5. Flashing details shall conform to SR specifications.

SCOPE OF WORK
Simon Roofing and Sheet Metal Corp Shall:
Preparation/Demolition
• Conduct a pre-job meeting with owner/owner's representative(s)to review jobsite safety issues,
loading/unloading areas, planned production schedule and review any/all owner related
construction concerns.
• Install all necessary Safety Equipment to perform the Scope of Work.
• Inspect existing wood deck substrate at areas suspect of dry rot/deterioration and replace as
needed
o NOTE: This quotation does not include the removal/replacement of any dry
rotted/deteriorated wood deck.If any dry rotted/deteriorated metal deck is located during
the visual inspection phase it will be removed/replaced on a per unit basis.Please refer
to the Line Item Cost Chad below for unit pricing
• Remove and retain existing metal coping cap for re-use upon project completion
• One(1)set of existing refrigeration are currently installed too close to the roof surface and will
need to be raised by BP Arco Approved Mechanical contractor in order to be properly flashed by
Simon Roofing. The cost for the BP Arco approved contractor to perform this work scope is not
included in this quotation
Installation
1. Furnish and install one(1)layer W EPS fan-fold insulation mechanically fastened over existing
roofing assembly per manufacturers standard specifications
2. Furnish and install tapered insulation(up to 175 square feet)to aid in achieving positive drainage
3. Furnish and install a.060 Mil mechanically attached TPO roofing system as per the manufacturer
specifications including all components for a complete watertight assembly
4. Fumish and install new aluminum retrofit drain inserts at existing sump drain locations and flash
in per manufacturers standard specifications
5. Properly encapsulate existing wood sleepers at HVAC unit with TPO and tie-in to the new roofing
assembly
6. Heat-weld all seams according to manufacturer specifications
7. Check all seams to ensure proper welds
8. Furnish and install new TPO coated metal thru-wall scupper sleeves and tie-in to the new roofing
assembly per manufacturers standard specifications

9. Furnish and install new wood blocking set on protection pads under all pipes as needed
Completion
• Remove and dispose of all debris related to the roofing project.
• Upon completion of project,and after receipt of final payment,furnish owner with a Contractor 2
year Leak Free warranty and a TPO Manufacturer 10-year material warranty,

We submitted a Permit Application for a roofing repair project located at 807 S. Lincoln
Avenue/BP #5719—Arco on or about 11-17-2014. 1 have enclosed a check in the correct
amount/balance due of$472.75 for the fee.
Would someone, please, contact me when the permit has been generated? We would
like to begin work as soon as possible, weather permitting. I will look for the original in
the mail.
