They noted some issues I was unaware of. We will have to remove the existing canopy and add some tube steel members that do not appear to have been installed correctly. The attachment to the concrete wall behind the limestone veneer will require a redesign of the brackets used to attach the canopy due to the change in depth. Andrew Vanleeuwen has tentatively committed to have this re-design done by Friday, May 19. Once we receive the design, we can remove the canopy and have it remodeled per the engineers' instructions. Are there any requirements for permits associated with obstructing the sidewalk to remove the canopy that we need to obtain? If so, could you fax me the forms? I apologize for the delays involved in addressing this issue. Upon entering the building, Captain Doyle found light smoke in the structure at which time he requested Chief Officer response. Upon my investigation, we found several areas where small incipient fires had been started. Entry had been made through a boarded up rear window. Mr. Ed Thomas, the premise maintenance keeper, responded to the incident and indicated there had obviously been intruders living in and had partied in this building since one month ago when he had last checked it. He indicated he had to continually secure the facility from intruders. He also indicated he has not been paid for his service for the past one year. As indicated by you in a memo to Dennis Dixon (attached), we have been paying for the lease telephone line to maintain the fire alarm system. This bill, to the best of my knowledge, has not been paid by Mr. Man Yeung Tee. The fire alarm system existence is imperative in protecting this premise from such acts in which we have witnessed. As outlined in the Uniform Fire Code, Section 2.201(b), "All buildings which constitute a fire hazard or are otherwise dangerous to human life, or which in relation to existing use, constitutes a hazard to safety or health or public welfare, by reasons of inadequate maintenance, dilapidation, obsolescence, fire hazard, disaster damage or abandonment as specified in this code or any other effective ordinance, are, for the purpose of this section, unsafe buildings. All such unsafe buildings are hereby declared to be a public nuisance and shall be abated by repair, rehabilitation, demolition or removal in accordance with the procedures specified Larry Glenn Page 2 July 2, 1991 in Chapters 4 through 9 of the Uniform Code of Abatement of Dangerous Buildings. Fire Department will identify the exact location of the Knox Box for the contractor. Please pick up an application for the Knox Box at the Fire Department. 3. The gate shall be provided with a "Knox Lock" padlock to allow immediate fire department access when secured. Please have the contractor pick up an application for the Knox Lock padlock at the fire department. 2. The site office (trailer) obstructs the fire hydrant located near the east end of the building. Appropriate temporary signage and a large diameter hose extension with a gated 2½ inch wye shall be provided to allow the immediate use of the hydrant should a fire occur in the vicinity. The fenced area noted above shall not enclose the fire hydrant. 3. Contact the fire department prior to demolition of the existing wood-frame stairs to ensure provisions are made for alternate ingress/egress should an emergency occur. 4. Minimum 4A-20BC fire extinguishers shall be provided on each floor. 5. Not less than one temporary standpipe system shall be provided for use during demolition and construction. Attached hoses and nozzles shall be accessible so that all portions of the building under construction or demolition are within 30 feet of the hose. 6. Under current state law and municipal code provisions, the city manager has authority to waive or reduce various fees. If you concur with the provisions included in this letter, please notify me in writing on or before Thursday, December 10, so I may present it at the December 15 Council meeting. Most likely, Council will either approve or deny the recommendation at that meeting. While the city manager has some discretion on these matters, I intend to present to Council a recommendation for their consideration. In regards to fees for building permits, plans review, fight-of-way permit and plumbing inspections, the fee was estimated to total $20,413.21 for those services. I will recommend those be reduced by fi fry-percent, for a revised amount not to exceed $10,206.60. Requests for waiving landfill fees are reviewed on a case by case basis, in accordance with utility ordinances governing solid waste tipping rates. The current fee for disposing of one ton of waste at the landfill is $76.80. The estimate tonnage for this project is 600 tons, which would total $46,080. As you already aware, the City has established a 50% rate consideration for some charitable organizations. However, extending that rate reduction to the Housing Authority and specifically to building debris from this project will require an amendment to include the promotion of affordable housing as a specific charitable consideration. On review of your request, I will recommend City Council adopt such an amendment. This would reduce the rate to $23,040. The final request concerns the utilization of a City owned roll-offdumpster for the disposal of debris at the site, and the subsequent transporting of the dumpster to the landfill. This request cannot be accommodated at this time. I suggest you contact a private waste hauler to provide this service. As a private hauler will be transporting debris and waste from the site to the landfill, a documentation system will need to be established so the attendant at the landfill facility will charge the lesser rate. Port Angeles, WA 98362 RE: Temporary Property Use Dear Virginia, As we discussed today on the phone Trilogy Group is respectfully requesting the use of your two lots located on 2~d Street, directly behind and above the Lee Hotel site. The purpose of the access is to remove fallen tree debris on the bluff, load it into a dumpster, haul it away and to lower materials intended for bluff stabilization down to the back of the building. With your approval, we propose to use your site July 6~ and 7~. Due to the limited access to the bhck of the building, we propose to use a boom extension crane for two days, to lift the debris off the bluff instead of dragging or otherwise damaging the sensitive surface vegetation using the City alley access to the east. It is our objective to minimize the impact on the soils, on and around the bluff, and request your assistance in pursuing this objective. Trilogy Group is fully insured and holds you as the property owner harmless from any damage, which occurs to the bluff during our operations. Furthermore, should any vegetation or blufftopography be disturbed during Trilogy Group's two days of use; we will make all reasonable and professional efforts to restore the bluff and its protective vegetation, back to its original or better condition. Even though Trilogy Group will make every effort to minimize its impacts during operations, we can make no guarantees or warrantees to potential future events or bluff deterioration, which may occur as a result from, or due to other factors, after our operations are finished. The Lee Hotel restoration project is fully permitted through the City of Port Angeles building and planning departments. Attached is a copy of our contractor's license and insurance certificate for your records and peace of mind that we will act in a professional manner. FIRE DEPARTMENT DATE: December 28, 1998 TO: File - Lee Hotel Remodel, 112 West 1st Street FROM: Dan McKeen, Fire Marshal ~.,_~)J F3 RE: Temporary Standpipe/Adjacent Fire Hydrant This memo is provided to clarify and document recent conversations with Kurt Honey regarding the Fire Department's requirement for a temporary standpipe system and access to the adjacent fire hydrant. The Fire Department will require a single temporary wet standpipe system to be placed inside the most centrally located stairway enclosure. The standpipe system shall be provided with "approved" inch and one-half diameter hoses and nozzles, capable of extending to within 30 feet of any portion of the building. A valve shall be provided on each floor to allow immediate activation of this system should a fire occur. When the building's fire sprinkler system is installed and operational, the temporary wet standpipe system can be deleted with an "approved" dry standpipe system installed in each stairway enclosure. The adjacent fire hydrant shall remain immediately accessible at all times. A clear unobstructed area shall be maintained to allow the use of a hydrant wrench. Two 50 feet sections of"approved" 3-inch fire hose can be attached to the fire hydrant to accomplish immediate access to the hydrant's ports as the location of the trailer and fence obstruct the ports. Hydrant signage shall be posted on the outside of the trailer to indicate location of the hydrant for emergency responders. It should be noted that the crib wall is independent of the building to the east; this building is supported by a post and beam foundation approximately 2 feet from the wall. Based on my review, I find that the loads imposed on the crib wall by the sand castle are less than loads imposed by the vehicles under normal parking lot use conditions. As such, it is my opinion that the construction of the sand castle will not negatively impact the safety of the wall. Please give me a call if you have any questions. Chain and lock fire sprinkler riser control valves. 2. Trim out control valve access area and provide cover for the elevator sprinkler control valve. 3. Sprinkler heads in the open basement room exceed the required distance from the north wall. 4. Surface mounted flourescent lights will obstruct the spray pattern of numerous concealed sprinkler heads in the basement. 5. All fire sprinkler heads shall be uncovered and operational prior to occupancy. 6. Sidewall sprinkler heads in the stairways are obstructed by surface mounted flourescent lights. 7. Change intermittent temperature heads in recessed areas within the mezzanine to standard temperature heads. 8. All fire sprinkler auxiliary drains shall be provided with access covers and signage. 9. Ensure sprinkler heads in the trash room are not obstructed by surface mounted flourescent lights. 10. Ensure sprinkler heads in the west stairway entrance are not obstructed by surface mounted flourescent lights. 11. When parts are available, ensure the fire department connections do not extend past the sprinkler system's post indicating valve. Prior to final accsptan~ the above items will need correction.
