Parcel Number 063000-513390 i Dear Mr. Griffin and Others Concerned, BACKGROUND AND SCOPE OF WORK On June 18th, 2014, Trent Adams, EIT, of NTI Engineering and Land Surveying (NTI) completed an inspection of the underpinning system, adjoining slope, drain lines, and the substructure of the house located at 705 Caroline Street. On June 30th, 2014, Steve Luxton, Senior Geotechnical Engineer, completed a follow-up inspection to verify site conditions and preliminary observations. That property lies within the City of Port Angeles on a high bluff along the shoreline about 0.25 miles east of the Port Angeles city pier. The subject house is located on an erosional bluff along the shoreline of Port Angeles Harbor. A structural underpinning and tie back system was installed at the house in the spring of 2005 after a landslide of 2004 destabilized the foundation of the house. A portion of the underpinning and tieback and slope revetment system were constructed on land owned by the City of Port Angeles. In allowing the use of the City property for construction, the City of Port Angeles called for follow-up inspections on the underpinning system to ensure the on-going stability and safety of the house and its occupants. This inspection is pursuant to that requirement. SUMMARY OF FINDINGS A careful inspection of the underpinnings, slope revetment system, drain lines, and substructure of the house showed the system to be intact and functioning as intended. The inspection did not reveal signs of strain or subtle rotation that would suggest downslope motion of the house or underpinnings was in progress. In the late winter of 2014, a notable groundwater driven slide occurred within close proximity of the subject parcel to the east, however, that slide did not affect the Griffin parcel or the I 1 underpinning system. LIMITATIONS OF THIS INSPECTION AND REPORT The observations and conclusions of this report apply only to the subject property and they are not transferable to nearby or adjoining property. This report is the sole property of the property owners and may be used by others only with their permission. NTI Engineering and Land Surveying warrants that this inspection and report were diligently and conscientiously completed in accordance with the practice of professional engineering and according to the principals of geologic science. Grantor acknowledges that all risks may not be eliminated by the design and engineering of the existing and proposed mitigations. 3. Grantor has decided to continue to maintain the Property, and agrees on behalf of Grantor and Grantor's heirs, successors, and assigns to accept any and all risks of loss, damage and injury associated with (a) use of the Property and Improvements; (b) development or construction on the Property; or (c) any combination thereof. B. WAIVER Grantor, on behalf of Grantor and Grantor's heirs, successors, and assigns, hereby WAIVES, RELINQUISHES, AND COVENANTS not to assert any present and future claims against the City, whether known or unknown, for any loss or damage occurring either on or off 2 the Property, which relates to or arises out of the risks described in Section A or the issuance of any permit or approval by the City contemplated by or in any way associated with this Covenant. C. INDEMNITY 1. Grantor, on behalf of Grantor and Grantor's heirs, successors, and assigns, agrees to indemnify, hold harmless and defend the City and its officers, agents and employees from and against any and all claims, losses, costs, and damages sustained by anyone, at any time, including, without limitation, personal injury, death, property, damage, loss of use, and attorneys' fees, arising out of or relating to the City's involvement in permit issuance, inspection, or approval of any development or alteration of the Property, and/or any of the risks described in Section A above, including, but not limited to, acts arising out of the negligence of the Grantor, any and all persons and entities involved in the design, construction, or maintenance of improvements to the Property, and other third parties, as well as acts arising out of the concurrent negligence of the City or its employees or agents and the foregoing parties. 2. Grantor, on behalf of Grantor and Grantor's heirs, successors, and assigns, specifically and expressly agrees to waive Grantor's and Grantor's heirs', successors', and assigns' immunity under industrial insurance, Title 51 of the Revised Code of Washington, to the extent necessary to provide the City with a full and complete indemnity from claims made by employees of Grantor or Grantor's heirs, successors, and assigns. Grantor, on behalf of Grantor and Grantor's heirs, successors, and assigns, specifically and expressly agrees that such waiver of immunity and indemnity were mutually negotiated by the parties. D. DUTY TO INFORM 1. Grantor, on behalf of Grantor and Grantor's heirs, successors, and assigns, agrees to inform all subsequent heirs, successors, and assigns of the Property or any portion of it (a) the Property are in or contain a potential landslide area, and (b) that there are risks associated with the Property and development thereon, as described above in Section A. 2. Grantor, on behalf of Grantor and Grantor's heirs, successors, and assigns, agrees to provide a copy of this Covenant to any prospective purchaser, lessor, occupant, or assignee of the Property prior to closing, assignment, tenancy, or occupancy. 3. Grantor, on behalf of Grantor and Grantor's heirs, successors, and assigns, agrees to inform all subsequent heirs, successors, and assigns of the Property of: (a) any conditions or prohibitions on development of any features of the Property and, natural or constructed, or of the development, that will require monitoring, maintenance, modification, or replacement; and (b) to help identify any such conditions, prohibitions, or features, the advisability of reviewing the City records of any City permit or approval for development or alteration of the Property. 3 E. INSURANCE Grantor, on behalf of Grantor and Grantor's heirs, successors, and assigns, agrees to maintain in force, personal injury insurance of $500,000 per incident, and name the City as an Additional Insured on such policy, providing annual proof of maintenance of the same. Such insurance must specifically cover, without limit, those risks described in Section A. F. RIGHT OF WAY PERMIT Grantor, on behalf of Grantor and Grantor's heirs, successors, and assigns, agrees to apply for and secure a right -of -way permit from the City for use of Area B on an annual basis. This may include, without limitation, inspection of the Property by the City or its designated agent(s), confirmation of adequate insurance coverage, and requests for further information and/or reports pertaining to the Property. G. RECORDING This Covenant shall be recorded in the real estate records of the Office of Records and Elections of Clallam County, Washington. H. RUNNING COVENANT The parties intend that this Covenant touches and concerns the land, shall run with the land, and be binding on Grantor and on Grantor's heirs, successors, and assigns. I. SEVERABILITY If any provision of this Covenant is held invalid, the remainder of the Covenant is not affected. Dear Mr. Griffin: Background At your request, Bill Payton, Engineering Geologist, with NTI Engineering and Surveying, Inc. conducted a geotechnical inspection of the above-mentioned property during March and April of 2004. The purpose of this inspection was to visually observe the bluff at the subject property and make recommendations regarding the mitigation of a slide that occurred on the bluff on January 7, 2004. NTI conducted a similar geotechnical evaluation of the property in September of 2003 for a potential buyer. Site Description The subject high bluff property is located at 705 Caroline Street in Port Angeles, overlooking the Strait of Juan De Fuca (Figure 1). The property is bounded on the north by the bluff and waterfront trail, on the south by East Caroline St., and on the east and west by residential homes. The house at the property was built in the early 1900's and is situated at the top of the bluff and extends a short distance down the face of the bluff (the bluff having been benched to accept the apparent perimeter concrete and interior post and block foundation). Access to the north side of the house is via a stairway down the bluff on the west side of the house and then along a catwalk along the north side of the house underneath the overhanging deck. The house has been added onto over the years. The yard is vegetated with grass and some ornamental trees and shrubs. There is an out building at the west end of the property. The yard slopes gently towards the bluff. The bluff face is vegetated with young trees (predominantly Alder) and brush except where the slide stripped off the vegetation. Other older slide scars were noticed on the bluff face that has since grown over. Many of the trees on the bluff face have curved '1 -------------- ------ ----------- , , "pistol butt" trunks indicating that shallow down slope creep of the surface soils is occurring. The bluff face soils were damp, however no springs were noticed on the bluff face. The toe of the bluff is protected from marine erosion by the waterfront trail and a rock bulkhead. The bluff is about 79 feet high with an average slope angle of about 40- 42 degrees, although, the lower bluff is steeper at about 50 degrees. The house is equipped with roof gutters and downspouts. There was some debris noticed in the gutters, It appears that the downspouts are connected to a tight line drain that extends down the west side of the bluff to the base of the bluff. A tight line running do.woJhe east side of the bluff does not extend all the way to the toe of the bluff and cneed~t0:6,e~exle.naed. This line may belong to the neighbor east of the subject property. It appears that the house is founded on a combination of shallow concrete footings with concrete stem walls, and post and block supports. Several bottle jacks were noticed in the crawl space suggesting that some settlement of the house has occurred and that attempts have been made to level the structure. Readily yisible portions of the concrete foundation and interior brick chimney were observed for the presence of settlement cracks during the previous evaluation and none were noticed. This may indicate that there has been relatively little settlement of the concrete portions of the foundation over the years. Some undermining of the north footing was noticed at the east and west ends of the house during the previous evaluation. This is possibly a result of soil creep and erosion/sloughing on the steep slope adjacent to the foundation. Observations made after the January 7 slide revealed that more of the footing had been undermined because of the slide. The slide was triggered by a reported waterline break due to freezing. Geologic Conditions The SCS Soil Survey of Clallam County maps the soil in the upland area of the subject property as the Clallam-Hoypus gravelly sandy loams (14), and the bluff as Dystric Xerorthents, extremely steep (19). The Clallam-Hoypus soil formed in compact glacial till and glacial outwash on hills and outwash terraces. This soil is typically composed of silty sand or silty gravel. Permeability ranges from rapid to very slow. There may be a perched water table during the wet season. The hazard of water erosion is slight. Plant cover is necessary in order to reduce erosion. The Dystric Xerorthents soil is found on coastal bluffs and formed in colluvium derived dominantly from glacial drift. This soil is also typically composed of silty sand or silty gravel. Permeability of this soil is moderately rapid and,the hazard of water erosion is severe. Plant cover is necessary in order to reduce erosion. The Washington State Department of Ecology's Coastal Zone Atlas of Washington maps the upland area of the subject property as Everson Sand (Qes?) and the bluff as Undifferentiated Pleistocene sediments (Qu):The Everson Sand is typically composed of sand, pebbly sand, and silt deposits. These deposits are located adjacent to stream valleys draining the Olympic Mountains. They also uncjerlie fan-shaped landforms that grade into the flat topography of the glaciomarine drift. This suggests deposition in a deltaic environment at the time of the Everson high sea level stand. 2 ----------- I The Undifferentiated Pleistocene sediments consist of glacial and nonglacial deposits and are mapped where detailed analysis was unavailable due to steepness of the bluff or other factors. The Atlas maps the stability of the bluff as Intermediate and of the upland as Stable, Visual observations made at the site are generally consistent with the above descriptions. Stratified sand and pebbly sand deposits were observed, as well as occasional thin silt deposits. The exposed sediments on the bluff face below the house are very compact. Soil logs from previous geotechnical work performed at the hospital, located about two blocks east of the subject property, indicate that standard penetration test blow counts for the soils approximately 20' below the top of the bluff and lower are 50+. The soil logs are also generally consistent with the above descriptions. Conclusions and Recommendations The house at the subject property has been perched on the bluff for nearly a hundred years with apparently only minor settlement. There have been slides on the bluff in the past, and there is a potential for future slides. There is no way to predict if or when a slide may occur, or how large the slide may be. It seems that the main trigger for slides on the bluff in the area of the subject property is the presence of excessive water in the soil. Generally when slides occur in the area, it is in the wet season during or after prolonged or heavy rainfall events, or in this case, a waterline break. Generally, the slides that occur in the area are relatively shallow, removing a foot or so of the surface soil and vegetation, leaving a bare slope face, which usually becomes re-vegetated naturally over time. Another potential trigger for a landslide would be the occurrence of an earthquake of sufficient magnitude to destabilize the bluff. This area is in seismic zone three of a four level scale with one being the lowest and four being the highest. Given the age of the house. its location on the edge of the bluff, and its apparent lack of a deep foundation, it is probably not economically feasible to retrofit the house in order to be capable of withstanding such an earthquake. The focus of this report is limited only to the mitigation of the most recent slide. If further studies regarding earthquake analysis and retrofitting are desired. you are encouraged to obtain such studies. The existing drainage system should be t roughly inspected and cleaned. The 1'3"-'~ inspection should determine the adequ cy of the system to divert all runoff to the ~ J base of the bluff via the tightline drain There should be no leaks in the system. The system should be repaired or modified as necessary to capture all runoff such that no runoff is allowed to flow over the face of the bluff uncontrolled. The drainage system should be inspected and cleaned at least once a month. 2. Vegetation on the bluff face provides stability to the bluff by reducing erosion and removing some of the water in the soil. Vegetation growth should be encouraged and enhanced as necessary. Bare areas should be re-vegetated. The use of erosion control mats (ecm) or turf reinforcement mats (trm) may be necessary to 3 help vegetation get started on the steep slope and to increase the strength of the root zone once vegetation has been reestablished, Due to the steepness of the slope, a rather strong mat is recommended. Based on past conversations with ecm/trm suppliers, at least a North American Green C350 ecm/trm or equivalent is recommended (See Appendix), Please refer to the three Washington State Department of Ecology (DOE) publications included in this report for further information on slope stabilization measures and plants. Also, an experienced landscape architect can assist in this matter. 3. Under current building codes, it would probably not be possible to build a house as close to the bluff as the subject house is. Typically, a house that is constructed close to a bluff would be founded on deep piles in order to protect the house from being undermined in case of a landslide. It appears that the subject house is founded on shallow footings. Thus the subject house is not as well protected as a properly constructed modern house would be. We recommend that at least the north footing line be supported by some type of pile foundation system in order to support the existing undermined areas ofthe footing and to mitigate potential further undermining from possible future slides similar to the slide that just occurred. It also would be beneficial to somehow mechanically strengthen the slope below the footing for some distance so that if another slide did occur, the slide might be prevented from reaching the north footing line. A structural engineer should be consulted with regards to these issues. These are just ideas and the structural engineer should be consulted for specifics and for the best solution to the situation. Please see the Appendix for soil property values that may be of assistance. 4. With respect to yard maintenance, it would be best to plant deep-rooted drought tolerant native plants and to not use irrigation (remembering that excessive water in the soil can lead to bluff instability). Any water lines/irrigation lines that are close to the bluff should be abandoned and capped off back away from the bluff. The reason for this is that a broken water line could trigger a slide on the bluff or a slide that undermines a water line could cause the line to break and create a worse situation. Water lines should be protected from freezing. Also, at times when the house will be vacant, it may be beneficial to turn off the water at the meter thus reducing the potential for a broken water line triggering another slide. This report cannot guarantee that another slide will not occur at the subject property, even if all of the recommendations in this report are implemented. Further remedial work may be necessary in the future. Some items to consider are placing a retaining wall at the toe of the slope to bring the lower bluff to a less steep angle and hard facing the entire slope with soil nails and shotcrete. The three DOE publications included with this report are published by the Washington State Department of Ecology (DOE) entitled: "Slope Stabilization and Erosion Control Using Vegetation", "Vegetation Management: A Guide for Puget Sound Bluff Property Owners" and "Surface Water and Groundwater on Coastal Bluffs". I also was at the site on March 26 and April 7 with Bill Payton, L.E.G., ~ Engineering Geologist at NTI, to observe the completed project. The final embedment depth of the vertical and horizontal anchors were less than ~ initially specified on the original drawing submittal due to the soil density but considered acceptable as reported and indicated on the February, 2005 revisions of Sheets 2 and d 3. The acceptance is based on previous conversation with Bill Payton. =::-. The originally designed grade beams at the western portion of the residence ~ tying the vertical anchors at the external to and along the wall line were also replaced ~ by a retaining wall and shotcreted structural slab as indicated on the revised plans. This structural slab is tied into the vertical and horizontal foundation anchors. The eastern section delineated by the step in thickness to the distribution and cover slab that ties the horizontal anchors together is not integral with the vertical foundation anchors and grade beam under the existing foundation wall. Although the eastern cover and distribution slab comes in contact with the foundation wall above and structural slab to the west but the lack of rebar tie at these locations would potentially allow this section to act somewhat separately (still re:l...ained by the horizontal ties) in an event that may cause future slope instability. The design and review were limited to the support of the north foundation wall of the residence after the slide. Although this involved the determination of contributing vertical loads to the new Chance anchor supports and support beam at the western portion, no other review of the existilig structure was performed. This remaining existing structure was not negatively affected by the slide once the foundation was stabilized, but that is not a verification of adequacy of the other portions of the residence. Since it is an existing structure not affected by the recent event there was not requirement to provide such a review.
